摘要: 目的:分析S100A7蛋白的表达量与宫颈鳞癌患者各个临床病理指标之间的关系,探讨该蛋白在宫颈鳞癌中的表达与肿瘤分化、侵袭、转移之间的关系。方法:选取2016年5月至2021年4月因宫颈鳞癌在青岛大学附属医院妇科行手术切除的标本49例,采用免疫组化的方法,对S100A7蛋白进行半定量分析,收集整理宫颈鳞癌患者的临床病理信息,运用统计学方法分析S100A7蛋白表达量与各临床病理参数之间的关系。结果:宫颈鳞癌中,S100A7蛋白的表达量与肿瘤分化、淋巴结转移有关,与发病年龄、肿瘤大小、FIGO分期、脉管转移无关。结论:S100A7蛋白与宫颈鳞癌分化、淋巴结转移有关,S100A7蛋白表达上调可能与宫颈鳞癌发生、进展、侵袭、转移有关。
Abstract: Objective: To analyze the relationship between the expression of S100A7 protein and various clini-copathological indicators in patients with cervical squamous carcinoma, and to explore the rela-tionship between the expression of this protein in cervical squamous carcinoma and tumor differ-entiation, invasion and metastasis. Methods: Forty-nine specimens with surgical resection for cer-vical squamous carcinoma in the Department of Gynecology, Affiliated Hospital of Qingdao Univer-sity were selected from May 2016 to April 2021, and the S100A7 protein was analyzed semi-quantitatively by immunohistochemistry. The clinicopathological information of patients with cervical squamous carcinoma was collected and collated. The relationship between S100A7 protein expression and various clinicopathological parameters was analyzed by using statistical methods. Results: In squamous carcinoma of the cervix, the expression of S100A7 protein was associated with tumor differentiation and lymph node metastasis, but not with age at onset, tumor size, FIGO stage and vascular metastasis. Conclusions: S100A7 protein was associated with cervical squamous car-cinoma differentiation and lymph node metastasis, and up-regulation of S100A7 protein expression may be related to cervical squamous carcinoma occurrence, progression, invasion and metastasis.

1. 引言

宫颈癌是女性最常见的恶性肿瘤之一,该疾病的发病率和死亡率都排在第四位 [1] 。不同于发达国家中的下降趋势,在中国,宫颈癌的发病率和死亡率仍在上升 [2] 。这种现象也使得我们对宫颈癌越来越重视,并正在采取有针对性的措施来扭转这一趋势。S100A7蛋白,也称银屑病素,由Madsen等人 [3] 首次在银屑病患者的角质形成细胞中发现。值得关注的是,S100A7蛋白在多种不同肿瘤组织中差异性表达,被证实与肿瘤细胞的增殖、迁移、侵袭密切相关,特别是参与了肿瘤的早期进展 [4] [5] ,在口腔鳞癌中,S100A7可以作为该疾病发生和早期进展的阳性标记物 [6] 。研究S100A7蛋白在宫颈鳞癌组织中的表达情况,再与各项临床指标相结合,可以为宫颈鳞癌的诊断、治疗及预后提供指导和帮助。

2. 材料与方法

2.1. 研究对象

2016年5月至2021年4月因宫颈鳞癌于青岛大学附属医院妇科行手术治疗的患者。

2.2. 实验方法及结果判定

通过免疫组化的方法对宫颈鳞癌组织切片进行染色,在200倍镜下随机选取5个视野,对病理切片中S100A7蛋白的表达情况进行半定量分析。根据细胞的染色强度进行评分,无染色为0分,淡黄色为1分,棕黄色为2分,深棕黄或深褐色为3分,每个视野下所选区域的染色强度根据以下方式进行计算:无染色区域所占百分比 × 0分 + 淡黄色区域所占百分比 × 1分 + 棕黄色区域所占百分比 × 2分 + 深棕黄或深褐色区域所占百分比 × 3分,染色越深得分越高。分别计算每个组织切片中随机选择的5个视野的染色分值,5个视野染色分值的平均值即为该切片的染色分值。

3. 结果

3.1. S100A7蛋白在宫颈鳞癌中的表达

3.2. 宫颈鳞癌中S100A7蛋白的表达量与各临床病理参数之间的关系

S100A7蛋白在分化程度不同的宫颈鳞癌组织中的表达具有差异性,差异有统计学意义(P < 0.05)。S100A7蛋白的表达量在高分化组(2.18 ± 0.21)、中分化组(1.62 ± 0.35)、低分化组(1.28 ± 0.38)依次降低,且在高分化组、中分化组、低分化组的表达量两两比较差异均具有统计学意义(P < 0.05)。

S100A7蛋白的表达量在宫颈鳞癌有淋巴结转移组中(1.84 ± 0.40)高于宫颈鳞癌无淋巴结转移组(1.51 ± 0.40),差异具有统计学意义(P = 0.040)。

S100A7蛋白的表达量在年龄 ≥ 45岁组(1.57 ± 0.46)与年龄 < 45岁组中基本一致,差异不具有统计学意义(P = 0.988)。

S100A7蛋白的表达量在肿瘤 ≥ 4 cm组(1.74 ± 0.35)高于肿瘤 < 4 cm组(1.54 ± 0.42),在宫颈鳞癌FIGOII-III期组(1.77 ± 0.44)高于宫颈鳞癌FIGOI期组(1.53 ± 0.40),在有脉管转移组(1.55 ± 0.48)高于无脉管转移组(1.54 ± 0.33),但是S100A7蛋白在这些组中的表达差异都不具有统计学意义(P值分别为0.236、0.109、0.893)。见表1

4. 讨论

为了明确S100A7在宫颈鳞癌中的表达情况,我们将样本中S100A7蛋白的表达量与临床病理信息结合进行分析。研究发现,S100A7蛋白在分化程度不同的宫颈鳞癌组织中的表达具有差异性,差异有统计学意义。S100A7蛋白的表达量在高分化组、中分化组、低分化组依次降低,且表达量两两比较差异均具有统计学意义。对食管鳞状细胞癌的研究也表明,S100A7蛋白高表达的食管鳞癌组织比S100A7蛋白低表达的食管鳞癌组织分化程度更高 [7] 。也有研究表明,S100A7蛋白促进了肺腺癌向肺鳞状细胞癌的转分化 [8] 。因此,我们考虑S100A7是促进肿瘤分化的重要因子。本研究中,S100A7蛋白的表达量在宫颈鳞癌有淋巴结转移组中高于宫颈鳞癌无淋巴结转移组,差异具有统计学意义。既往的研究表明,对于肿瘤的侵袭、转移过程,S100A7蛋白的作用在不同肿瘤组织中是不同的,例证如下。一方面,Alowami等人的研究表明,S100A7蛋白的表达量在皮肤原位癌中高于浸润性癌组织 [9] ;Leygue等人的研究表明,S100A7蛋白的表达量在乳腺癌的原发灶组织高于转移灶组织 [10] ,基于S100A7蛋白的表达量在肿瘤的转移过程中下调,考虑S100A7蛋白可能与抑制肿瘤的侵袭及转移有关。另一方面,在对肺鳞癌组织及食管鳞癌组织的研究中发现,S100A7蛋白的表达量在原发灶组织中低于转移灶组织 [11] [12] ;在宫颈癌组织中,S100A7蛋白可以增强细胞间充质性质,诱导上皮-间充质转化,促进宫颈癌细胞的迁移、侵袭和转移 [13] 。因此,考虑S100A7蛋白有促进肿瘤细胞转移和侵袭的作用。通过上述研究发现,在不同的肿瘤组织中,S100A7蛋白有着不同的作用,它可以促进肿瘤细胞增殖、侵袭、转移或抑制肿瘤增殖、侵袭、转移。本研究中,S100A7蛋白的表达量在有淋巴结转移的宫颈鳞癌组织中更高,因此认为,与食管鳞癌、卵巢癌等疾病相似,在宫颈鳞癌组织中,S100A7蛋白对肿瘤的增殖和侵袭发挥了促进作用。本研究发现,S100A7蛋白的表达量与患者年龄之间无明显相关关系,考虑S100A7蛋白的表达量与年龄无关,这与目前该蛋白在多种肿瘤中的研究结果一致。此外,本研究中S100A7蛋白的表达量在肿瘤 ≥ 4 cm组高于肿瘤 < 4 cm组,在宫颈鳞癌FIGOII-III期组高于宫颈鳞癌FIGOI期组,在有脉管转移组高于无脉管转移组,但是S100A7蛋白在这些组中的表达差异都不具有统计学意义,即S100A7蛋白的表达量与这三个指标都无明显的相关性。但是,S100A7蛋白已经在口腔鳞状细胞癌 [6] [14] 、食管鳞状细胞癌 [7] [11] 、肺鳞癌 [12] [15] 等多种肿瘤组织中被证明与肿瘤细胞的迁移、侵袭、转移等过程有关,在本实验中,S100A7蛋白与淋巴结转移也有一定的相关性,由于S100A7蛋白在肿瘤组织中的作用复杂多样,其具体的分子机制还需要其他研究来进一步明确,因此,我们不能排除多种因素相互影响导致实验结果出现偏差的可能性,不能最终判定在宫颈鳞癌中S100A7蛋白的表达与肿瘤大小、肿瘤分期、脉管转移之间绝对不存在相关性,其相互关系尚待确定。

5. 结论

S100A7蛋白与宫颈鳞癌分化、淋巴结转移有关,S100A7蛋白表达上调可能与宫颈鳞癌发生、进展、侵袭、转移有关。
