摘要: 本文报道一例胆囊梭形细胞型未分化癌;患者,男,63岁,20天前因间断性上腹部疼痛就诊于当地医院,行B超提示:胆囊占位;为求进一步治疗于我院就诊;影像检查提示胆囊癌可能,术前考虑胆囊癌(T3期)可能,术中冰冻提示恶性梭形细胞肿瘤,故行胆囊癌根治术及肝部分切除术。大体示肿瘤位于胆囊腔内,呈息肉样生长,切面灰白灰红色,实性,质中;部分肝脏切面灰红色,质软,未见明显异常。镜下,肿瘤主要由梭形细胞组成,局部见少量高分化腺癌成分,核分裂象易见;免疫组化染色:梭形细胞同时表达波形蛋白(Vimentin)和细胞角蛋白(cytokeratin, CK)。胆囊梭形细胞型未分化癌是一种相对罕见的胆囊原发恶性肿瘤,临床症状不典型,发病率较低,术前诊断困难,明确诊断依赖于病理,恶性程度高,需予以重视。
Abstract: This article reports a case of undifferentiated spindle cell carcinoma of the gallbladder. The patient, a 63-year-old male, consulted a local hospital 20 days ago due to intermittent epigastric pain, and ultrasound showed that the gallbladder was occupied. In order to seek for further treatment, he consulted our hospital, and the image suggested that gallbladder cancer might be possible; pre-operative consideration was given to possible gallbladder cancer (stage T3) and intraoperative freezing suggestive of malignant spindle cell tumor, so radical cholecystectomy and partial hepa-tectomy for gallbladder cancer were performed. Grossly, the tumor was located in the lumen of the gallbladder, with polypoid growth, grayish-white and grayish-red in color, solid and medium in texture; part of the liver was grayish-red in color, soft in texture, and no obvious abnormality was seen. Under the microscope, the tumor was mainly composed of spindle cells, with a small amount of highly differentiated adenocarcinoma locally, and nuclear schizophrenia was easy to be seen. Immunohistochemical staining: the spindle cells expressed both vimentin and cytokeratin. Undif-ferentiated spindle cell carcinoma of the gallbladder is a relatively rare primary malignant tumor of the gallbladder with atypical clinical symptoms, low incidence, difficult preoperative diagnosis, de-finitive diagnosis relying on pathology, and high degree of malignancy, which needs to be taken se-riously.

1. 引言

胆囊梭形细胞型未分化癌是一种相对罕见的恶性肿瘤,因其主要由束状排列的梭形细胞构成,故在组织学形态上易与平滑肌肉瘤、恶性纤维组织细胞瘤及胃肠道间质瘤等间叶来源恶性肿瘤混淆;又因其除梭形细胞成分外,常伴有腺癌成分,与癌肉瘤的组织学构成相似,故难以鉴别;本文报道了一例胆囊梭形细胞型未分化癌患者的临床病理特征及鉴别诊断,并复习相关文献,全面认识该疾病,避免误诊。

2. 病例报告

患者,男,63岁,既往体健,无高血压、高血脂、糖尿病及冠心病等病史。20天前因上腹部疼痛就诊于当地医院,呈间断发作的钝痛,偶伴恶心,无呕吐,无发热,无皮肤巩膜发黄;癌胚抗原(carcinoembryonic antigen, CEA) 0.50 ng/mL,肿瘤标志物糖类抗原(carbohydrate antigen, CA) 199 27.12 U/mL;行B超提示:胆囊占位;期间给予消炎利胆药物(具体不详)治疗后症状好转。后患者家属为求进一步治疗,遂于我院就诊,以“胆囊占位”收之入院。入院后患者查体,上腹部压痛阳性,无反跳痛,Murphy征阴性;2022年11月15日于我院行上腹部MRI平扫 + DWI + 增强扫描示:1) 胆囊占位性病变,考虑肿瘤性病变,胆囊癌可能(倾向于T3期可能);2) 胆囊窝临近肝实质异常所见,考虑炎性改变。术前考虑胆囊癌(T3期)可能,术中冰冻检查提示恶性梭形细胞肿瘤,故行胆囊癌根治术及肝部分切除术。后患者因身体状况较差未进行化疗,于2023年1月16日去世。

病理检查:大体示:胆囊一枚,长8 cm,径3 cm,壁厚0.5~4 cm,胆囊腔内见灰白色息肉样肿物一枚,体积6.5 cm × 4.5 cm × 3.5 cm,肿物表面光滑,切面灰白灰红色,实性,质中;部分肝脏组织,体积8 cm × 5 cm × 3 cm,切面灰红色,质软,未见明显异常。术后病理示(图1):大部分肿瘤细胞呈束状排列;肿瘤细胞呈梭形,胞浆嗜酸性,胞核圆形、卵圆形,染色质粗颗粒状,可见核仁,核分裂像多见;局部可见高分化腺癌成分;并可见鳞状细胞化生;伴局灶出血坏死;肿瘤细胞侵透胆囊壁全层,侵及胆囊肌周结缔组织(肝脏侧);可见脉管内癌栓,肿瘤细胞侵犯神经。免疫组化染色:波形蛋白(Vimentin)、CK7、P63阳性,CK、CK5/6部分阳性,P40、CK19、SMA、Desmin阴性,Ki-67增殖指数梭形细胞成分约80%、腺癌成分约40%。

病理诊断为胆囊梭形细胞型未分化癌。

3. 讨论

原发性胆囊癌最常见的组织学类型为腺癌。胆囊未分化癌(Undifferentiated carcinoma of the gallbladder, UCG)是一种相对罕见的胆囊的恶性肿瘤,发生率仅占原发性胆囊癌的3.4% [1] ;世界卫生组织(World Health Organization, WHO)消化系统肿瘤分类将胆囊未分化癌分为四种亚型,分别为梭形和巨细胞型、类似破骨细胞的巨细胞型、小细胞型及结节型或小叶型,其中梭形和巨细胞型最常见 [1] ,本例病例即为此型,由于其名称过长,故本文以梭形细胞型未分化癌代称。复习已报道明确为胆囊梭形细胞型未分化癌26例(表1) [1] - [9] ,女性多发,发病年龄范围55~80岁,发病年龄高峰60~65岁,临床常以上腹部疼痛为首发症状,可伴有发热、腹泻、食欲不振、黄疸等,CEA与CA19-9多在正常范围内 [1] [4] [6] ;胆囊未分化癌CT及B超表现为腔内异质性肿块 [4] ,本例MRI示胆囊腔内混杂信号包块影,增强扫描呈渐进性持续强化,其内可见斑片状无强化区。胆囊未分化癌多表现为胆囊腔内息肉样肿物,平均最大径5.7 cm;胆囊腺瘤或乳头状瘤也表现为胆囊腔内息肉样肿物,但其为良性病变,不会发生肝脏侵犯及淋巴结转移 [10] 。因此,Hyo Jun Park等人建议,当发现胆囊腔内息肉样肿物 > 4 cm时,应考虑将未分化癌作为鉴别诊断,应行术中冰冻检查明确肿物性质,以便下一步手术方案的确定 [1] 。复习文献发现,26例患者中有22例在肿瘤组织中发现有腺癌和/或粘膜上皮高级别上皮内瘤变,可见胆囊梭形细胞型未分化癌常伴发上皮内瘤变,因此,在临床实践中需对胆囊梭形细胞型未分化癌的不同部分进行充分取材,以便发现肿瘤组织中散在的腺癌成分,对疾病做出正确诊断。仅有2例提及肿瘤中存在破骨细胞样巨细胞,说明破骨细胞样巨细胞在胆囊梭形细胞型未分化癌中并不常见;17例病例展示了肿瘤的免疫组化结果,其中有16例梭形细胞局灶表达上皮标记物(包括EMA、CK、CEA和CAM5.2),可见上皮标记物在梭形细胞区域表达可帮助诊断胆囊梭形细胞型未分化癌。此外,在该肿瘤组织中还可见到鳞状细胞分化。

本病例梭形细胞区域的Ki-67指数高于腺癌区域,与Tomotaka Akatsu等人的发现相同,预示其预后不良 [6] 。Yoshitaka Furuya等人报道了一例梭形细胞型胆囊未分化癌伴内皮细胞分化,其在形态学上没有明显证据,但肿瘤细胞血管内皮生长因子(VEGF)、CD31、CD34、FVIII的表达证明了内皮细胞分化,患者于术后五个月死亡 [2] ;由于目前仅有一例报道,故存在内皮细胞分化对预后是否有影响尚不清楚。大部分报道的梭形细胞型未分化癌病例局灶可有腺癌成分,且与未分化癌之间有过渡区,这些过渡区提示未分化癌可能起源于这些腺癌的去分化或化生 [4] [6] ;Ying Xiao等人对胆囊未分化癌的梭形细胞成分和腺癌成分分别进行基因检测;在梭形细胞组分中检测到TP53、TERT、ARID2和CDH1基因发生突变,并且发现CCND1、FGF19和MET基因发生了扩增;在腺癌成分检测到102个体细胞突变和一个扩增,其中包括在梭形细胞成分中发现的四个突变和一个扩增(CCND1) [3] ,基于此认为两种不同的成分可能具有相同的起源。

鉴别诊断:1) 癌肉瘤:胆囊癌肉瘤极为罕见,常发生于中老年患者,女性多于男性,肿瘤组织由上皮和间叶两种成分构成,其中上皮肿瘤包括腺癌、鳞癌或两者均有,间叶肿瘤类型多样,常见的有纤维肉瘤、骨肉瘤、平滑肌肉瘤、血管肉瘤等,两种肿瘤成分不等,共同组成癌肉瘤 [11] 。与胆囊未分化癌相比,癌肉瘤体积更大,起源于全能干细胞,分别分化为上皮细胞和肉瘤细胞。未分化癌梭形细胞表达细胞角蛋白(CK)有利于与癌肉瘤区分。2) 发生在胆囊的恶性梭形细胞肿瘤:如平滑肌肉瘤、恶性纤维组织细胞瘤和胆囊源性胃肠道间质瘤,平滑肌肉瘤具有特征性的“雪茄样”核,特异性表达SMA;恶性纤维组织细胞瘤常有奇异型瘤巨细胞,并混合有数量不等的圆形组织细胞样细胞;胆囊源性胃肠道间质瘤CD117、DOG-1、CD34阳性。且其梭形细胞均不表达CK。

胆囊梭形细胞型未分化癌预后差,中位生存期仅5.2个月,易出现邻近器官侵犯及淋巴结转移,尤其是肝脏侵犯;目前主要的治疗方案为手术治疗联合术后化疗。

胆囊未分化癌为发生在胆囊的少见癌症,本文通过报道一例梭形细胞型胆囊未分化癌,并复习相关文献,进一步认识其临床病理特征,避免误诊。
