老年患者结核性中耳炎一例报告
Tuberculous of Otitis Media in an Elderly Patient: A Case Report
摘要: 目的：报告一例老年结核性中耳炎(TOM)病例，并探讨其诊断与治疗策略。方法：回顾性分析1例71岁女性TOM患者的临床资料，包括临床表现、影像学、手术及病理结果。结果：患者行乳突切除术、鼓室成形术及听骨链重建，术后恢复顺利。病理确诊为TOM。术后给予标准抗结核治疗，随访中未见复发。结论：老年TOM少见且易误诊。早期识别并联合手术与规范化抗结核治疗，可有效控制感染，保留功能并减少并发症。
Abstract: Objective: To report a rare case of tuberculous otitis media (TOM) in an elderly patient and evaluate diagnosis and treatment strategies. Methods: Clinical data of a 71-year-old female with TOM were retrospectively analyzed, including presentation, imaging, surgery, and pathology. Results: The patient underwent mastoidectomy, tympanoplasty, and ossicular reconstruction. Postoperative recovery was uneventful. Pathology confirmed TOM. Standard antitubercular therapy was given, with no recurrence during follow-up. Conclusion: TOM in elderly patients is rare and easily misdiagnosed. Early recognition and combined surgery with standardized ATT can control infection, preserve function, and reduce complications.
