基于“正虚伏毒”理论探析肺结节“结癌转化”的防治思路
Exploring Prevention and Treatment Strategies for “Nodule-to-Cancer Transformation” Based on the Theory of “Deficiency of Vital Energy with Latent Toxin”
摘要: 肺结节作为肺癌的重要前驱病变,具有起病隐匿、进展缓慢、发病后方显的临床特点,其“结癌转化”过程是中西医防治中的关键环节。文章以中医“正虚伏毒”理论为核心,将肺结节的病机演变过程分为三期:初始阶段为“正气受损,邪气内伏于肺络,结节将成未成”,此阶段正气虽虚而不甚,邪气潜藏未显;形成阶段则“正气进一步暗耗,津血失调,痰瘀互结”,痰瘀有形之邪胶着肺络,积聚渐成;至癌变阶段,则呈现“正气溃败,伏毒久蕴,癌毒酿生”的危重态势,伏毒郁蒸,癌毒始生,病势转险。在防治策略方面,结合中医辨证论治体系,提出分期干预思路:初始期以补益肺脾肾、调摄精神及传统功法等非药物疗法为主,旨在扶正御邪;形成期则应消痰祛瘀、解毒散结,以控制结节进展;癌变期须解毒散结与扶正固本并举,旨在遏制癌毒扩散,挽救机体功能。文章旨在构建一套契合中医理论、兼具阶段针对性的肺结节防治策略,为临床早期识别与干预提供理论依据,从而延缓甚至阻断其“结癌转化”进程,改善患者生存质量与预后。
Abstract: Pulmonary nodules, recognized as significant precursor lesions for lung cancer, are characterized by their insidious onset, slow progression, and delayed clinical manifestation following their initial development. The transition from nodule to malignancy marks a critical point in both Western and traditional Chinese medicine (TCM) approaches to prevention and treatment. This paper focuses on the TCM theory of “deficiency of vital energy with latent toxin” and categorizes the pathogenesis of pulmonary nodules into three distinct stages. The initial stage is characterized by “depleted vital energy” with pathogenic factors lurking within the lung meridians, during which nodules are in the process of formation. Although vital energy is diminished, it is not severely deficient, and the pathogenic factors remain latent, not yet manifesting clinically. During the formation stage, vital energy is further depleted, leading to imbalances in fluids and blood, while phlegm and blood stasis become intertwined. Tangible pathogens, including phlegm and stasis, adhere to the lung vessels, gradually accumulating and solidifying over time. In the stage of malignant transformation, the condition reaches a critical state marked by “collapse of vital energy”, prolonged accumulation of latent toxin, and the emergence of cancerous toxin. The latent toxin stagnates and heats up, ultimately giving rise to cancerous toxin, signaling the progression of the disease to a perilous stage. This paper proposes a phased intervention strategy for prevention and treatment, grounded in the pattern differentiation and treatment framework of TCM: The initial phase primarily involves non-pharmacological therapies, including nourishing the lungs, spleen, and kidneys, regulating the spirit, and incorporating traditional exercises, with the goal of enhancing the body’s resistance to pathogens; In the formation stage, the focus is on resolving phlegm, eliminating blood stasis, detoxifying, and dispersing nodules to inhibit their progression; In the cancerous phase, detoxification and dispersal of nodules should be combined with efforts to strengthen the body’s defenses and consolidate its foundations. This dual approach aims to limit the spread of cancerous toxins while restoring vital bodily functions. This paper aims to develop a lung nodule prevention and treatment strategy rooted in TCM theory, incorporating stage-specific approaches. It offers a theoretical foundation for early clinical identification and intervention, with the goal of delaying or even halting progression to lung cancer, thereby improving patient quality of life and prognosis.
