摘要: 溃疡性结肠炎(Ulcerative Colitis, UC)是一种慢性非特异性肠道炎症性疾病,其治疗理念正从单纯抑制炎症转向重塑肠道稳态。尽管传统药物在控制症状方面取得了明显进展,但临床应答率有限、复发率高以及长期安全性问题仍是突出的临床瓶颈。肠道微生态失衡不仅是UC发病的核心驱动因素,亦是影响药物疗效的重要变量。随着对“菌群–免疫–代谢”轴认识的深化,微生态制剂与传统药物的联合应用已从辅助支持发展为具有明确协同增效机制的治疗新策略。文章系统梳理该联合疗法的临床证据与分子机制,以期为优化UC个体化治疗提供理论依据与实践指导。
Abstract: Ulcerative colitis (UC) is a chronic nonspecific inflammatory bowel disease, and its treatment concept is shifting from simply suppressing inflammation to remodeling intestinal homeostasis. Although conventional medications have made notable progress in symptom control, limited clinical response rates, high recurrence rates, and long-term safety issues remain prominent clinical bottlenecks. Intestinal dysbiosis is not only a core driver of UC pathogenesis but also an important variable affecting drug efficacy. With a deeper understanding of the “microbiota-immune-metabolism” axis, the combined application of microecologics and conventional drugs has evolved from adjunctive support into a novel therapeutic strategy with clearly defined synergistic mechanisms. This article systematically reviews the clinical evidence and molecular mechanisms of this combination therapy, aiming to provide a theoretical basis and practical guidance for optimizing individualized treatment of UC.
