Abstract: Liver transplantation (LT) is the only effective treatment for end-stage liver disease and acute liver failure. The global demand for liver transplant patients continues and far outstrips the supply of liver donors. Due to the widespread use of opioids, the proportion of donors infected with hepatitis C virus (hepatitis C virus, HCV) is rising and getting younger, making it imperative to fully utilize all transplantable organs in a shortage of organs. Currently, highly effective and well tolerated direct action antiviral (DAA) therapies are available to treat HCV infection and eliminate HCV in allografts in more than 95% of cases. This article reviews the safety and efficacy of HCV positive donor liver transplantation in patients with negative recipients.
