经皮经肝胆囊穿刺引流术在胆囊炎急性发作治疗中的研究进展
Research Progress of Percutaneous Transhepatic Gallbladder Drainage in the Treatment of Acute Attack of Cholecystitis
摘要: 随着科技的发展,硬件的升级和多年临床经验的积累,医学得到的发展越来越快。急性胆囊炎(acute cholecystitis, AC)作为临床中的常见疾病,其诊疗方案逐步完善。现今伴随着经皮经肝胆囊穿刺引流术(percutaneous transhepatic gallbladder drainage, PTGBD)技术的不断发展完善,使得急性胆囊炎在微创介入诊疗方面拥有了新的选择,并为外科手术提供了安全的保障。PTGBD的适应症及禁忌症,优缺点,安全性及有效性目前已有指南作为导向,但针对PTGBD后的拔管时间及择期手术时间至今没有提出明确的观点。故本文不仅综合介绍了部分PTGBD的在急性胆囊炎的临床诊疗的进展,也为PTGBD的拔管时间及择期手术时间选择提供了部分汇总,以供临床参考。
Abstract: With the development of science and technology, the upgrade of hardware and the accumulation of many years of clinical experience, medicine has been developing faster and faster. Acute cholecysti-tis (AC) is a common disease in clinic, its diagnosis and treatment plan is gradually improved. In re-cent years, with the continuous development and improvement of percutaneous transhepatic gallbladder drainage (PTGBD), acute cholecystitis has a new choice in minimally invasive interven-tional diagnosis and treatment, and provides a safe guarantee for surgery. At present, there are guidelines for the indications and contraindications, advantages and disadvantages, safety and ef-fectiveness of PTGBD, but there is no clear point of view on the time of extubation and elective op-eration after PTGBD. Therefore, this paper not only comprehensively introduces the progress of some PTGBD in the clinical diagnosis and treatment of acute cholecystitis, but also provides a par-tial summary for the choice of selective operation time of PTGBD for clinical reference.
