摘要: 光疗作为一种非药物干预手段,在抑郁症治疗中展现出多靶点、低副作用的优势。本文系统综述了光疗在不同类型抑郁症(包括季节性情感障碍、非季节性抑郁、双相抑郁、脑卒中后抑郁及围产期抑郁)中的疗效及机制研究进展。其核心作用机制涉及视网膜–下丘脑通路调控昼夜节律、激活黑视素神经节细胞(ipRGCs)调节情绪相关神经环路、促进5-羟色胺释放及抑制神经炎症反应。疗效受光照强度(推荐2500~10,000 lux)、波长(蓝光与近红外光各具优势)、治疗时长及个体差异影响。尽管光疗安全性高,但仍存在眼部刺激、转躁狂风险等局限性。未来需结合多组学技术优化个体化治疗方案,并探索与数字疗法的联合应用。
Abstract: Phototherapy, as a non-pharmacological intervention, has shown advantages of multi-target effects and low side effects in the treatment of depression. This article systematically reviews the efficacy and mechanism research progress of phototherapy in different types of depression, including seasonal affective disorder, non-seasonal depression, bipolar depression, post-stroke depression, and perinatal depression. Its core mechanisms involve regulating the circadian rhythm through the retina-hypothalamus pathway, activating melanopsin retinal ganglion cells (ipRGCs) to modulate mood-related neural circuits, promoting serotonin release, and inhibiting neuroinflammatory responses. The efficacy is influenced by light intensity (recommended 2500~10,000 lux), wavelength (blue light and near-infrared light each have their advantages), treatment duration, and individual differences. Although phototherapy is highly safe, it still has limitations, such as eye irritation and the risk of mania. Future research needs to combine multi-omics technology to optimize personalized treatment.
