摘要: 胰十二指肠切除术(PD)是治疗胰头及壶腹周围疾病的标准术式,其中胆肠吻合(HJ)是关键重建步骤,直接影响术后胆漏、狭窄等并发症风险及患者长期预后。近五年来,随着微创技术、ERAS理念及新型器械的发展,该领域研究已从技术安全性的初步验证,转向循证指导下的操作标准化、并发症精准防治及长期功能结局优化。此文系统梳理2019~2024年间重要进展,归纳核心共识,以便给临床实践和未来研究方向提供专业参考。
Abstract: Pancreaticoduodenectomy (PD) is the standard procedure for treating diseases of the pancreatic head and periampullary region, in which Hepaticojejunostomy (HJ) serves as a critical reconstructive step that directly affects the risk of postoperative complications such as bile leakage and stricture, as well as long-term patient prognosis. Over the past five years, with the advancement of minimally invasive techniques, the integration of the ERAS concept, and the development of novel surgical instruments, research in this field has shifted from initial validation of technical safety toward evidence-based standardization of procedures, precision in complication prevention and management, and optimization of long-term functional outcomes. This article systematically reviews key developments between 2019 and 2024, summarizes core consensus findings, and aims to provide a professional reference for clinical practice and future research directions.
