肛肠外科专科医师规范化培训体系的探索与实践

Exploration and Practice of the Standardisation Training System for Specialist Training in Colorectal Surgery

摘要: 自2013年起，我国全面推行“5 + 3”医学人才培养体系，有效提升了我国医师的平均临床水平。然而，在技术进步与亚专科化发展的进程中，该体系已难以担负起培养青年专科医师的重任。因此，本文以肛肠外科为例，参考美国等发达国家的成熟经验，提出一套符合我国国情的肛肠外科专科医师培训方案。该方案以海军军医大学附属长海医院肛肠外科为实践平台，整合理论、临床、科研三大模块，并进一步细化为分病种轮转、结肠镜教学、CRC多学科诊疗(MDT)等内容，强调培训过程的标准化与可复制性。近五年来，学员出科考核通过率超97%，结肠镜培训通过率达98%，学员总体满意度超过98%，教学成效良好。

Abstract: Since 2013, China has comprehensively implemented the “5 + 3” medical training system, effectively enhancing the average clinical proficiency of its physicians. However, amid technological advancements and the progression of subspecialisation, this framework has proven inadequate for cultivating young specialist surgeons. Consequently, this paper proposes a tailored specialist training programme for colorectal surgeons in China, drawing upon established practices from developed nations such as the United States and using colorectal surgery as a case study. The programme takes the Department of Colorectal Surgery of Changhai Hospital affiliated to the Naval Medical University as the practice platform, integrates the three major modules of theory, clinic, and research, and further refines them into sub-disease rotations, colonoscopy teaching, and multidisciplinary diagnostic and therapeutic (MDT) of CRC, emphasizing the standardisation and replicability of the training process. Emphasis is placed on standardising and replicating the training process. Over the past five years, trainee pass rates for departmental assessments have exceeded 97%, colonoscopy training pass rates have reached 98%, and overall trainee satisfaction has surpassed 98%, demonstrating excellent teaching outcomes.
