摘要: 目的：分析自体富血小板血浆(PRP)联合重组人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)凝胶治疗下肢静脉性溃疡的临床效果。方法：选取2023年9月~2024年7月西京医院烧伤与皮肤外科门诊换药中心收治的90例下肢静脉性溃疡患者，采用随机数字表法分为对照组和研究组，每组各45例。对照组采取常规的rhGM-CSF凝胶换药治疗，研究组在对照组基础上联合使用PRP治疗，分析比较两组疗效。结果：研究组总有效率91.1%，明显高于对照组68.8% (P < 0.05)。研究组创面愈合速率为0.49 ± 0.07 cm2/w，较对照组0.26 ± 0.06 cm2/w快(P < 0.05)，愈合时间为27.3 ± 3.7天，较对照组51.2 ± 2.8天缩短(P < 0.05)。研究组不良反应发生率为8.8%，较对照组15.6%降低(P < 0.05)。结论：PRP联合rhGM-CSF凝胶治疗下肢静脉性溃疡效果显著，可加速创面愈合，值得临床推广。
Abstract: Objective: To analyze the clinical effect of autologous platelet-rich plasma (PRP) combined with recombinant human granulocyte macrophage colony-stimulating factor (rhGM-CSF) gel in the treatment of venous lower extremity ulcers. Methods: A total of 90 patients with venous lower extremity ulcers admitted to the Outpatient Dressing Center of the Department of Burn and Dermatology Surgery of Xijing Hospital from September 2023 to July 2024 were selected and randomly divided into a control group and a study group, with 45 cases in each group. The control group received conventional rhGM-CSF gel dressing treatment, and the study group used PRP treatment on the basis of the control group. The efficacy of the two groups was analyzed and compared. Results: The total effective rate of the study group was 91.1%, which was significantly higher than that of the control group (68.8%) (P < 0.05). The wound healing rate of the study group was 0.49 ± 0.07 cm2/w, which was faster than that of the control group (0.26 ± 0.06 cm2/w) (P < 0.05), and the healing time was 27.3 ± 3.7 days, which was shorter than that of the control group (51.2 ± 2.8 days) (P < 0.05). The incidence of adverse reactions in the study group was 8.8%, which was lower than that of the control group (15.6%) (P < 0.05). Conclusion: PRP combined with rhGM-CSF gel is effective in treating venous ulcers of lower extremities and can accelerate wound healing, which is worthy of clinical promotion.
