摘要: 再设计出院项目(project re-engineered discharge, RED)模式作为一种院后延续性护理策略,衔接着医院与家庭护理,旨在优化出院教育指导,降低再入院率和医疗成本,减少并发症的发生,提高护理质量,提升患者过渡期的生活质量。本文综述了RED模式的概述以及在糖尿病患者延续性护理中的应用现状,为国内RED模式在糖尿病患者延续性护理的临床实践提供循证依据。
Abstract: The Re-Engineered Discharge (RED) program, as a post-hospitalization transitional care strategy, bridges hospital care with home-based care. It aims to optimize discharge education and guidance, reduce readmission rates and healthcare costs, minimize complications, improve care quality, and enhance patients’ quality of life during the transition period. This review summarizes the overview of the RED model and its current application status in transitional care for diabetic patients, providing evidence-based support for the clinical implementation of the RED model in diabetes transitional care in China.
