从“暗瘀”论治儿童肺炎支原体肺炎探析

摘要: 本文探讨了“暗瘀”理论在儿童肺炎支原体肺炎(MPP)诊疗中的应用。基于中医理论及现代医学研究,分析了“暗瘀”(一种无明显临床表征的血液运行不畅的病理状态)及其在MPP病程中的必然性,指出患儿肺气虚弱、气滞、痰热阻滞、热灼津亏等因素均可导致“暗瘀”。现代医学研究显示,MPP与凝血异常、血栓形成相关,与中医血瘀理论相契合。活血防瘀应贯穿肺炎支原体肺炎治疗全程,不同阶段采用针对性干预:早期疏风清热、凉血活血;中期清热解毒、化痰逐瘀;恢复期益气养阴、佐以活血。“暗瘀”理论将中医“治未病”理念与临床结合,通过干预潜在血瘀状态改善预后,为MPP防治提供了整合策略,有望补充西医在预防血栓并发症中的不足。
Abstract: This paper explores the application of the “Hidden Stasis” theory in the diagnosis and treatment of Mycoplasma Pneumoniae Pneumonia (MPP) in children. Based on traditional Chinese medicine (TCM) theories and modern medical research, it analyzes “Hidden Stasis” (a pathological state of poor blood circulation without obvious clinical manifestations) and its inevitability in the course of MPP, pointing out that factors such as weak lung qi, qi stagnation, phlegm-heat blockage, and heat-induced fluid deficiency in children can all lead to “Hidden Stasis”. Modern medical studies have shown that MPP is associated with coagulation abnormalities and thrombosis, which is consistent with the TCM theory of blood stasis. Activating blood circulation to prevent stasis should run through the entire treatment process of MPP, with targeted interventions at different stages: dispersing wind and clearing heat, cooling blood and activating blood circulation in the early stage; clearing heat and detoxifying, resolving phlegm and removing stasis in the middle stage; invigorating qi and nourishing yin, supplemented by activating blood circulation in the recovery stage. The “Hidden Stasis” theory combines the TCM concept of “preventive treatment” with clinical practice, improves prognosis by intervening in potential blood stasis, provides an integrated strategy for MPP prevention and treatment, and is expected to complement the limitations of western medicine in preventing thromboembolic complications.
