基于肝、脾、肾三脏论治月经过少的临床验案分析及机制探讨

Clinical Case Analysis and Mechanism Discussion on Treating Hypomenorrhea Based on the Liver, Spleen and Kidney

摘要: 月经过少是妇科常见病，病因复杂，与肝、脾、肾功能失调密切相关。本文通过分析三则典型医案(肝郁气滞、脾肾阳虚、肝肾阴虚证型)，探讨中医辨证论治月经过少的思路及疗效。结果显示，针对不同病机，分别采用疏肝解郁、温补脾肾、滋补肝肾法，均能显著改善月经量及伴随症状，说明精准辨证与经典方剂化裁结合可显著改善症状，调节激素水平，验证了中医个体化治疗的有效性。

Abstract: Hypomenorrhea is a common gynecological disease with complex etiologies, closely related to dysfunction of the liver, spleen, and kidney. This article explores the thinking and efficacy of TCM syndrome differentiation and treatment for hypomenorrhea by analyzing three typical medical cases (syndromes of liver qi stagnation, spleen-kidney yang deficiency, and liver-kidney yin deficiency). The results show that for different pathogenesis, methods of soothing the liver and relieving stagnation, warming and tonifying the spleen and kidney, and nourishing the liver and kidney are adopted respectively, all of which can significantly improve menstrual volume and accompanying symptoms. It indicates that the combination of precise syndrome differentiation and modification of classic prescriptions can significantly relieve symptoms, regulate hormone levels, and verify the effectiveness of TCM individualized treatment.
