摘要: 膝关节半月板损伤是临床常见运动损伤之一,其中内侧半月板后角至后内侧关节囊的移行区域的损伤称为半月板Ramp区损伤,是一种特殊类型的半月板损伤,常与前交叉韧带损伤伴发,由于其位置的特殊性,磁共振检出率较低,且常规前方关节镜入路不易发现。因损伤位于半月板红–红区血供丰富,处理后愈合率高,手术修补是治疗Ramp区损伤有效方法。主要手术方式包括全内缝合技术、由内向外缝合技术等。目前对于Ramp区损伤治疗文献报道较少,本文系统综述了Ramp区损伤的流行病学及生物力学特征、诊断方法、治疗技术等方面的研究进展,特别关注了全内缝合技术等方面的创新,以期为临床实践提供参考。
Abstract: Meniscal injuries of the knee are among the most common sports-related injuries. Among these, injuries occurring in the transitional zone between the posterior horn of the medial meniscus and the posteromedial joint capsule—known as meniscal ramp lesions—represent a special type of meniscal injury. These lesions are frequently associated with anterior cruciate ligament (ACL) injuries. Due to their specific location, ramp lesions exhibit low detection rates on magnetic resonance imaging (MRI) and are often missed during conventional anterior arthroscopic approaches. Located in the red-red zone of the meniscus, which is highly vascularized, ramp lesions have a high healing potential following surgical repair, making such intervention an effective treatment option. Primary surgical techniques include all-inside suturing and inside-out suturing. Currently, there is limited literature on the management of ramp lesions. This article systematically reviews recent advances in the epidemiology, biomechanical characteristics, diagnostic methods, and treatment techniques of ramp lesions, with a particular focus on innovations in all-inside suture repair, aiming to provide insights for clinical practice.
