冷球蛋白血症相关肾小球肾炎1例

摘要: 冷球蛋白血症通常与慢性丙型肝炎感染、自身免疫疾病、恶性肿瘤有关。症状通常表现为紫癜性皮疹、关节痛和全身无力的临床三联征。而无继发因素的为原发性冷球蛋白血症,已经有几份病例报告确定了丙型肝炎和冷球蛋白血症之间的关系。目前关于原发性冷球蛋白血症的病例少有报道,在此,我们报告了一例74岁男性,表现为血肌酐升高,但无丙型肝炎病史,发展为特发性混合性冷球蛋白血症。他接受了糖皮质激素和静脉用环磷酰胺治疗,肾功能明显有所改善。
Abstract: Cryoglobulinemia is usually associated with chronic hepatitis C infection, autoimmune diseases, and malignant tumors. Its symptoms typically present as a clinical triad consisting of purpuric rash, joint pain, and general weakness. Cryoglobulinemia without secondary causes is referred to as primary cryoglobulinemia. Several case reports have confirmed the relationship between hepatitis C and cryoglobulinemia. Currently, there are few reported cases of primary cryoglobulinemia. Herein, we report a case of a 74-year-old male who presented with elevated serum creatinine, had no history of hepatitis C, and developed idiopathic mixed cryoglobulinemia. He received treatment with glucocorticoids and intravenous cyclophosphamide, and his renal function improved significantly.
