郑永科“鱼渔并授”学术思想及临床经验总结
Zheng Yongke’s Academic Thought of “Teaching Both Knowledge and Methods” and Summary of Clinical Experience
摘要: 本文系统总结首届德阳市十大名中医郑永科老师“授人以鱼,亦授人以渔”(既传效方,更传思路)的学术思想与宝贵临床经验。郑老强调中医临证须以整体观念与辨证论治为核心,切忌读死书、死搬硬套。其经验特色突出表现为:善从经典理论(如脏腑经络学说)出发指导实践(如“肺开窍于鼻”治鼻窦炎、“足厥阴肝经绕阴器”治睾丸肿痛);精于方剂创新与活用,创制如“疏风清热、宣肺通窍”之鼻窦炎经验方(以银翘散合苍耳子散化裁)及疏肝理气、化瘀散结之睾丸肿痛方,疗效卓著;注重“三因制宜”,灵活变通经方应用(如麻黄附子细辛汤夏日使用之宜忌)。本文结合其近五十年验案,剖析其辨证思路与处方精要,彰显其“活学活用、守正创新”的学术精髓,为中医临床传承与发展提供重要借鉴。
Abstract: This paper systematically summarizes the academic ideas and valuable clinical experience of Professor Zheng Yongke, one of the Top Ten Famous Traditional Chinese Medicine Physicians of Deyang City, who adheres to the principle of “giving a fish and teaching how to fish” (i.e., imparting effective formulas while emphasizing the underlying rationale). Professor Zheng emphasizes that clinical practice in TCM must be guided by holistic concepts and syndrome differentiation, avoiding rigid bookishness and mechanical application. His distinctive clinical features are highlighted as follows: expertise in applying classical theories (such as the theory of zang-fu organs and meridians) to guide practice (e.g., treating sinusitis based on the principle that “the lung opens into the nose”, and treating testicular swelling and pain according to the pathway of the Liver Meridian of Foot-Jueyin circling the genital area); proficiency in innovating and flexibly applying formulas, such as creating an empirical formula for sinusitis by modifying Yinqiao Powder and Cang’erzi Powder to “dispel wind, clear heat, ventilate the lung, and open the orifices”, as well as developing a formula for testicular swelling and pain that soothes the liver, regulates qi, resolves stasis, and disperses nodules, both with remarkable efficacy; emphasis on “tailoring treatment to the individual, locality, and season”, and flexibly adapting classical formulas (e.g., considerations for using Mahuang Fuzi Xixin Decoction in summer). This paper analyzes his diagnostic reasoning and prescription essentials based on his clinical cases over the past fifty years, illustrating his academic essence of “flexible learning and application, upholding orthodoxy while innovating”, thereby providing significant reference for the inheritance and development of clinical TCM.
