刺络拔罐结合点刺放血三商穴治疗带状疱疹 后遗神经痛的效果分析

Analysis of the Efficacy of Point Pricking Bloodletting at the Sanshang Acupoints Combined with Pricking-Cupping Therapy for Postherpetic Neuralgia

摘要: 目的：对刺络拔罐结合点刺放血三商穴治疗带状疱疹愈合后神经疼痛的临床疗效进行探讨。方法：选取门诊收治的带状疱疹后遗神经痛患者，就诊时间为2024年1月24日至2024年12月24日期间，共计80例，随机将所有病例分成实验组与对照组(每组40例)。在治疗方式上，对照组为常规针灸治疗，实验组为刺络拔罐结合点刺放血三商穴治疗，并将两组患者的临床疗效进行对比分析。疗效分析方法包括睡眠质量评估、中医症状积分、视觉模拟评分法(Visual Analogue Scale, VAS)疼痛评分。结果：治疗后，实验组相对于对照组的VAS分数有明显下降(P < 0.01)；实验组在经过治疗后，与对照组相比，痒感、灼热感和触觉敏感度均有降低，两组之间的差异具有可比性(P < 0.01)；实验组在有效例数上明显多于对照组，有效率显著提高(P < 0.05)。结论：刺络拔罐结合点刺放血三商穴对带状疱疹后遗神经痛患者疗效显著，临床上值得提倡。

Abstract: Objective: To investigate the clinical efficacy of the therapy combining pricking and cupping with point pricking bloodletting at the Sanshang acupoints in the treatment of postherpetic neuralgia after herpes zoster healing. Methods: A total of 80 outpatients with postherpetic neuralgia who attended the clinic from January 24, 2024, to December 24, 2024, were enrolled as research subjects. All cases were randomly divided into an experimental group and a control group, with 40 cases in each group. For the treatment regimen, the control group was given conventional acupuncture and moxibustion therapy, while the experimental group received the combined therapy of pricking and cupping plus point pricking bloodletting at the Sanshang acupoints. The clinical efficacy of the two groups was compared and analyzed. The efficacy evaluation indicators included sleep quality assessment, TCM symptom score, and pain score measured by the Visual Analogue Scale (VAS). Results: After treatment, the VAS score of the experimental group decreased significantly compared with that of the control group (P < 0.01). After intervention, the experimental group showed reductions in pruritus, burning sensation, and tactile hypersensitivity compared with the control group, with statistically significant differences between the two groups (P < 0.01). The number of effective cases in the experimental group was significantly higher than that in the control group, and the total effective rate was notably improved (P < 0.05). Conclusion: The combined therapy of pricking and cupping with point pricking bloodletting at the Sanshang acupoints exerts significant clinical effects on patients with postherpetic neuralgia, and thus is worthy of promotion in clinical practice.
