Abstract: Limited research has explored the impact of physical activity (PA) and composite dietary antioxidant index (CDAI) on mortality outcomes in cancer populations. The purpose of this study was to investigate the separate and joint prognostic effects of PA and CDAI on survival rates among US cancer survivors. Data on cancer survivors (n = 2263, representing 22 million survivors) were obtained from the National Health and Nutrition Examination Survey (NHANES) between 2007 and 2016. PA was assessed using the self-reported Global Physical Activity Questionnaire (GPAQ), and CDAI evaluated total dietary antioxidant capacity through a panel of antioxidants—including vitamins (A, C, E), selenium, zinc, and carotenoids. The Cox proportional hazards model was used to evaluate the separate and joint relationship between the prognostic effects of PA and CDAI with mortality outcomes among cancer survivors. In the joint analysis, cancer survivors with sufficient PA and high CDAI demonstrated reduced all-cause mortality risk compared to those with insufficient PA or low CDAI. Concurrently, the joint effect of sufficient PA and high CDAI was associated with lower cancer mortality and non-cancer mortality. This finding may contribute to refining lifestyle intervention recommendations for cancer survivor populations.
