摘要: 目的:对慢性病患者药物素养的现状及影响因素进行范围综述,为慢性病患者用药安全管理和相关干预提供理论依据。方法:按照范围综述方法框架,确定相关研究问题,检索中国知网、万方、维普、中国生物医学数据库、PubMed、Web of Science、Embase和Cochrane Library,检索时限为建库至2025年5月。结果:最终纳入38篇文献。研究显示,慢性病患者药物素养水平处于中等偏下。常用的评估工具包括中文版药物素养调查问卷、药物素养调查量表及中文版高血压药物素养量表等。影响因素主要涉及社会人口学因素、疾病因素、药物相关因素及社会心理因素。结论:慢性病患者药物素养在不同疾病类型和人群中存在明显差异,而现有评估工具虽类型多样,但缺乏针对特定疾病及多病共存人群的标准化量表,且素养水平划分尚不统一。未来应进一步探讨药物素养的影响机制,开发适用于不同慢性病人群的评估工具,并为慢性病用药管理提供更具针对性的研究。
Abstract: Objective: To systematically map the existing evidence on the status and influencing factors of medication literacy among patients with chronic diseases through a scoping review, and to inform medication safety management and the development of targeted interventions. Methods: Following a scoping review methodology, relevant research questions were formulated, and a systematic literature search was conducted in CNKI, Wanfang Data, VIP, CBM, PubMed, Web of Science, Embase, and the Cochrane Library from inception to May 2025. Results: A total of 38 studies were included. The findings indicated that medication literacy among patients with chronic diseases was generally at a low-to-moderate level. Commonly used assessment tools included the Chinese version of the Medication Literacy Survey, the Medication Literacy Questionnaire, and the Chinese version of the Hypertension Medication Literacy Scale. Factors associated with medication literacy mainly involved sociodemographic characteristics, disease-related factors, medication-related factors, and psychosocial factors. Conclusion: Medication literacy varies across chronic disease conditions and patient groups. However, existing assessment instruments lack standardization, particularly for specific diseases and multimorbidity populations, and literacy level classifications remain inconsistent. Future research should further explore the influencing mechanisms of medication literacy, develop assessment tools applicable to different chronic disease populations, and provide more targeted evidence for chronic disease medication management.
