睫状体单针连续缝合术治疗外伤性睫状体离断一例并文献复习
Single-Needle Continuous Suturing for Traumatic Cyclodialysis Cleft: A Case Report and Literature Review
摘要: 探讨“单针连续缝合术”治疗外伤性睫状体离断的临床疗效及技术特点。方法:回顾性分析1例因外伤导致持续性低眼压、浅前房的外伤性睫状体离断患者的诊疗经过。患者接受经睫状体平坦部切口的房角镜辅助下睫状体单针连续缝合复位术。手术核心在于使用8-0聚丙烯缝线尾端导入30G针,对离断的睫状体进行一体化连续缝合固定。同时,检索中英文数据库相关文献进行复习。结果:术后患者睫状体离断裂隙完全闭合,前房形成,眼压由术前4.7 mmHg恢复并稳定在正常范围,最佳矫正视力从HM/BE提高至0.7。随访六个月无复发,未见严重并发症。文献复习提示,该术式在理论上具有减少眼内操作次数、提供均匀复位张力的潜在优势。结论:睫状体单针连续缝合术是一种安全、有效的治疗外伤性睫状体离断的改良手术方式,其设计优化了缝合流程,为临床提供了新的技术选择。
Abstract: Objective: To evaluate the clinical efficacy and technical characteristics of the “single-needle continuous suture technique” in the treatment of traumatic cyclodialysis cleft. Methods: We retrospectively analyzed the diagnosis and treatment course of a patient with persistent hypotony and shallow anterior chamber secondary to traumatic cyclodialysis. The patient underwent cyclodialysis cleft repair via the pars plana approach, assisted by gonioscopy, using the single-needle continuous suture technique. The core of the procedure involved the insertion of an 8-0 polypropylene suture into a 30G needle, followed by an integrated continuous suturing and fixation of the detached ciliary body. A literature review of relevant Chinese and English databases was also conducted. Results: Postoperatively, the cyclodialysis cleft was completely closed, the anterior chamber was well-formed, and the intraocular pressure (IOP) recovered from a preoperative level of 4.7 mmHg to the normal range and remained stable. The best-corrected visual acuity improved from hand motion (HM/BE) to 0.7. During the 6-month follow-up period, there was no recurrence or serious complications. The literature review suggests that this technique has potential advantages, such as reducing the number of intraocular maneuvers and providing uniform tension for repositioning. Conclusion: The single-needle continuous suture technique is a safe and effective modified surgical procedure for treating traumatic cyclodialysis cleft. Its design optimizes the suturing process and offers a new technical option for clinical practice.
