Abstract: Premature ovarian insufficiency (POI) is a reproductive endocrine disorder characterized by the decline of ovarian function occurring in women before the age of 40 years. It severely impairs female fertility and physical-mental health, while its pathological mechanisms remain to be fully elucidated. Cathepsins are a class of proteases predominantly localized in lysosomes, involved in various physiological and pathological processes including antigen presentation and extracellular matrix remodeling. Recent studies have revealed that cathepsins play pivotal roles in the pathogenesis of ovarian function decline, affecting ovarian function through modulating granulosa cell apoptosis, follicular atresia, oxidative stress, and inflammatory responses. However, their causal relationships and clinical translational value remain to be validated. This review summarizes the roles of cathepsins in premature ovarian insufficiency (POI) and the recent research progress in this field.
