Abstract: Neutrophils are core effector cells of the innate immune system, playing key roles in oral infection defense through phagocytosis, degranulation, and the formation of extracellular traps (NETs). Porphyromonas gingivalis, as a key pathogen in periodontitis, can evade neutrophil killing and exacerbate chronic inflammation via virulence factors, such as lipopolysaccharide (LPS), gingipains, and the Porphyromonas gingivalis peptidylarginine deiminase (PPAD). Notably, the interaction between the two is not confined to periodontal tissue destruction but may also participate in the pathogenesis of systemic diseases, such as rheumatoid arthritis and Alzheimer’s disease through immunomodulation. However, the molecular mechanisms of their interaction and its systemic implications remain insufficiently elucidated. This review systematically summarizes the modes of interaction between neutrophils and P. gingivalis, aiming to provide new insights into the prevention and treatment of periodontitis and related systemic diseases.
