累及弓上分支动脉主动脉病变腔内治疗的研究进展
Research Progress on Endovascular Therapy for Aortic Lesions Involving Supra-Aortic Branch Arteries
摘要: 胸主动脉疾病致死率高,TEVAR已成为切实可行的治疗方案,而弓上分支动脉重建是累及主动脉弓病变TEVAR治疗的关键,其远期通畅率直接影响患者预后。本文旨在检索近期以来关于主动脉弓上分支血管重建的高质量文献,试图梳理TEVAR弓上分支重建的开窗支架、分支支架、平行支架等主流技术的研究进展,分析各技术临床效果,探讨主动脉弓解剖、支架操作、术后管理及患者基础疾病等影响远期通畅率的关键因素,提出全流程优化策略,为临床术式选择及术后管理提供循证参考,并对未来研究方向进行展望。
Abstract: Thoracic aortic diseases are associated with high mortality, and thoracic endovascular aortic repair (TEVAR) has become the first-line treatment. Revascularization of supra-aortic branch arteries is critical in TEVAR for lesions involving the aortic arch, and long-term branch patency directly determines patient prognosis. This article reviews mainstream techniques for supra-aortic branch reconstruction in TEVAR, including fenestrated, branched, and parallel stent-graft technologies, and analyzes their clinical outcomes. It also discusses key factors affecting long-term patency, such as aortic arch anatomy, stent manipulation, perioperative management, and underlying comorbidities. Furthermore, comprehensive optimization strategies are proposed to provide evidence-based references for clinical procedure selection and postoperative management, with future research directions discussed.
