针灸联合中医穴位贴在踝关节骨折术后护理效果观察

Observation on the Nursing Effect of Acupuncture Combined with Traditional Chinese Medicine Acupoint Application in Postoperative Care of Ankle Fractures

摘要: 目的：观察针灸联合中医穴位贴在踝关节骨折术后护理中的应用效果，探讨其对患者关节功能恢复、疼痛缓解及并发症发生率的影响，为踝关节骨折术后优质护理提供临床参考。方法：选取新疆医科大学第六附属医院足踝外科收治的86例踝关节骨折术后患者作为研究对象，采用随机数字表法分为对照组和观察组，每组43例。对照组给予常规术后护理，包括生命体征监测、伤口护理、康复训练指导及基础健康宣教；观察组在对照组基础上，加用针灸联合中医穴位贴护理，连续干预4周。比较两组患者干预前后视觉模拟疼痛评分(VAS)、踝关节功能评分(AOFAS)，统计两组干预期间并发症(关节肿胀、僵硬、感染)发生率及护理满意度。结果：干预前，两组患者VAS评分、AOFAS评分比较，差异无统计学意义(P > 0.05)；干预4周后，两组患者VAS评分均较干预前显著降低，AOFAS评分均较干预前显著升高，且观察组VAS评分低于对照组，AOFAS评分高于对照组，差异均有统计学意义(P < 0.05)。观察组并发症发生率为4.65% (2/43)，显著低于对照组的18.60% (8/43)，差异有统计学意义(P < 0.05)。观察组护理满意度为97.67% (42/43)，显著高于对照组的83.72% (36/43)，差异有统计学意义(P < 0.05)。结论：针灸联合中医穴位贴应用于踝关节骨折术后护理，可有效缓解患者术后疼痛，促进踝关节功能恢复，降低并发症发生率，提高患者护理满意度，临床应用价值显著，值得推广。

Abstract: Objective: To observe the application effect of acupuncture combined with Traditional Chinese Medicine (TCM) acupoint application in the postoperative care of ankle fractures, and to explore its impact on patients’ joint function recovery, pain relief, and complication rates, thereby providing clinical reference for high-quality postoperative nursing for ankle fractures. Methods: 86 patients with ankle fractures admitted to the Department of Orthopedics of our hospital were selected as the study subjects. They were randomly divided into a control group and an observation group using a random number table, with 43 patients in each group. The control group received routine postoperative care, including monitoring of vital signs, wound care, rehabilitation training guidance, and basic health education. The observation group received additional nursing intervention with acupuncture combined with TCM acupoint application on top of the control group’s regimen for a continuous period of 4 weeks. The Visual Analogue Scale (VAS) score for pain and the American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score were compared between the two groups before and after the intervention. The incidence of complications (joint swelling, stiffness, infection) during the intervention period and nursing satisfaction were also recorded for both groups. Results: Before the intervention, there was no statistically significant difference in VAS scores or AOFAS scores between the two groups (P > 0.05). After 4 weeks of intervention, VAS scores significantly decreased and AOFAS scores significantly increased compared to pre-intervention levels in both groups. Furthermore, the observation group had a lower VAS score and a higher AOFAS score compared to the control group, with both differences being statistically significant (P < 0.05). The complication rate in the observation group was 4.65% (2/43), significantly lower than the 18.60% (8/43) in the control group (P < 0.05). Nursing satisfaction in the observation group was 97.67% (42/43), significantly higher than the 83.72% (36/43) in the control group (P < 0.05). Conclusion: The application of acupuncture combined with TCM acupoint application in the postoperative care of ankle fractures can effectively alleviate postoperative pain, promote the recovery of ankle joint function, reduce the incidence of complications, and improve patient satisfaction with nursing care. It demonstrates significant clinical application value and is worthy of promotion.
