实施性研究综合框架(CFIR)在VAP护理 质量改进中的应用效果及防控依从性影响

Application Effect of the Consolidated Framework for Implementation Research (CFIR) in VAP Nursing Quality Improvement and Its Impact on Prevention and Control Compliance

摘要: 目的：探讨实施性研究综合框架(CFIR)在呼吸机相关性肺炎(VAP)护理质量改进中的应用效果及对防控依从性的影响。方法：选取2024年1~12月我院行ICU机械通气患者100例为对照组，实施常规VAP护理；2025年1~12月ICU同类机械通气患者100例为观察组，基于CFIR开展VAP护理质量改进干预。分析比较VAP临床结局指标与防控依从性。结果：观察组VAP发病率、再插管率低于对照组，呼吸机及ICU住院天数更短，护理人员防控依从性更高，上述差异指标均具有统计学意义(P < 0.05)。结论：CFIR可有效提升VAP防控依从性，改善临床结局，具有较高的临床应用价值。

Abstract: Objective: To investigate the effect of the Consolidated Framework for Implementation Research (CFIR) on nursing quality improvement and prevention and control compliance of ventilator-associated pneumonia (VAP). Methods: A total of 100 patients who received mechanical ventilation in the ICU of our hospital from January to December 2024 were selected as the control group and received routine VAP nursing care. Another 100 similar patients undergoing mechanical ventilation in the ICU from January to December 2025 were enrolled as the observation group, and the intervention of VAP nursing quality improvement was carried out based on CFIR. The clinical outcome indicators of VAP and the compliance with prevention and control were analyzed and compared between the two groups. Results: The observation group had lower VAP incidence and re-intubation rates than the control group, shorter mechanical ventilation and ICU stays, and significantly higher prevention compliance. The differences in these indicators were statistically significant (P < 0.05). Conclusion: CFIR can effectively improve VAP prevention and control compliance, enhance clinical outcomes, and has high clinical application value.
