针刺干预血管性痴呆氧化应激–线粒体稳态–铁死亡相关机制的研究进展
Research Progress on the Mechanisms of Acupuncture Intervention in Vascular Dementia: Oxidative Stress, Mitochondrial Homeostasis, and Ferroptosis

摘要: 血管性痴呆是由脑血管病及其相关危险因素所致的获得性认知功能减退,慢性脑低灌注被认为是其重要病理基础。近年来研究表明,氧化应激、线粒体稳态失衡与铁死亡在血管性痴呆中相互耦联,共同参与神经元损伤、白质病变及认知障碍的发生发展。针刺作为防治血管性痴呆的重要干预手段,具有多靶点、整体调节的特点,可通过减轻氧化应激、改善线粒体能量代谢与质量控制、调节炎症及相关细胞死亡通路发挥神经保护作用。尤其是围绕Nrf2、TXNIP/NLRP3、AMPK、SIRT1/PGC-1α、NCOA4/FTH1及GPX4等信号的研究,为揭示针刺干预“氧化应激–线粒体稳态–铁死亡”网络提供了新的证据。本文围绕该连续病理轴,系统综述其在血管性痴呆中的作用及针刺干预的研究进展,并对现存问题与未来方向进行分析,以期为相关基础研究和临床转化提供参考。
Abstract: Vascular dementia (VaD) is an acquired cognitive disorder caused by cerebrovascular disease and related vascular risk factors, and chronic cerebral hypoperfusion is considered a major pathological basis. Recent studies have shown that oxidative stress, mitochondrial homeostasis imbalance, and ferroptosis are closely interconnected in VaD, and jointly contribute to neuronal injury, white matter damage, and cognitive decline. As an important intervention for VaD, acupuncture exerts multi-target and holistic regulatory effects, and may provide neuroprotection by reducing oxidative stress, improving mitochondrial energy metabolism and quality control, and modulating inflammation as well as cell death-related pathways. In particular, studies focusing on signaling pathways such as Nrf2, TXNIP/NLRP3, AMPK, SIRT1/PGC-1α, NCOA4/FTH1, and GPX4 have provided new evidence for understanding how acupuncture regulates the “oxidative stress-mitochondrial homeostasis-ferroptosis” network. This review summarizes the pathological relationship among these processes in VaD, discusses the research progress on acupuncture intervention, and analyzes current limitations and future directions, so as to provide references for further basic research and clinical translation.
