摘要: 三阳性乳腺癌(TPBC)是指雌激素受体(ER)、孕激素受体(PR)和人表皮生长因子受体2 (HER2)均呈阳性表达的乳腺癌亚型,具有独特的肿瘤行为和治疗反应。本文系统叙述了TPBC新辅助治疗的研究进展,尽管以蒽环类联合紫杉烷类为基础的化疗仍是标准骨架,但其疗效受限于耐药性和毒副作用。抗HER2靶向治疗能够显著提高HER2阳性乳腺癌患者的病理完全缓解率(pCR),新型抗体偶联药物(ADCs)在克服耐药和改善预后方面更是展现出突破性潜力。细胞周期依赖性激酶4/6 (CDK4/6)抑制剂为克服内分泌治疗耐药提供了新思路,新型口服内分泌治疗药物如完全雌激素受体拮抗剂(CERAN)等的开发也为激素受体(HR)阳性表达的乳腺癌患者带来了新的治疗可能。当前,多维度生物标志物分层治疗策略和液体活检等动态监测技术正推动TPBC新辅助治疗迈向精准化时代,真实世界数据库的建立与完善将进一步提升治疗的实用性与安全性。
Abstract: Triple-positive breast cancer (TPBC), defined by the co-expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2), exhibits distinct tumor biology and therapeutic behavior. This review systematically summarizes the recent advances in neoadjuvant systemic therapy (NST) for TPBC. Although anthracyclines combined with taxanes remain the standard chemotherapy regimen, their efficacy is frequently limited by drug resistance and toxicity. Anti-HER2 targeted therapy can significantly improve pathological complete response (pCR) rates in HER2-positive breast cancer. Novel antibody-drug conjugates (ADCs) have demonstrated breakthrough potential in overcoming resistance and improving prognosis. Given the intracellular crosstalk between ER and HER2 signaling pathways, cyclin-dependent kinase 4/6 (CDK4/6) inhibitors offer a promising strategy to overcome endocrine therapy resistance. In parallel, the development of novel endocrine agents provides new therapeutic opportunities for patients with hormone receptor (HR)-positive disease. Currently, the integration of multi-dimensional biomarker-driven strategies and dynamic monitoring technologies are propelling neoadjuvant systemic therapy into an era of precision medicine. The establishment and refinement of real-world databases for TPBC patients will further enhance the applicability and safety of these therapeutic approaches.
