1例复发型溃疡性结肠炎合并布氏杆菌病 和关节炎的病例分析

A Case Analysis of Recurrent Ulcerative Colitis Complicated with Brucellosis and Arthritis

摘要: 本文通过回顾性分析1例内蒙古自治区人民医院收治的56岁复发型溃疡性结肠炎合并布氏杆菌病及关节炎男性患者的临床资料并结合文献回顾，探讨其临床特征、诊断与治疗策略；该患者因发热、头痛、关节痛入院，同时伴有7年溃疡性结肠炎(ulcerative colitis, UC)病史，经血清学及影像学检查确诊为布氏杆菌病，予多西环素、利福平抗感染及美沙拉嗪、糖皮质激素抗炎联合治疗后病情减轻；结果提示，UC与布氏杆菌病症状重叠易导致误诊，关键是需详细询问流行病学史及针对性血清学检测，治疗需兼顾抗感染与控制炎症，个体化用药及多学科协作尤为重要。

Abstract: This study explores the clinical features, diagnosis, and treatment strategies of Ulcerative Colitis (UC) complicated with brucellosis and arthritis through a retrospective analysis of a 56-year-old male patient with recurrent UC admitted to the Inner Mongolia Autonomous Region People’s Hospital, combined with a literature review. The patient was admitted due to fever, headache, and arthralgia, with a 7-year history of UC. Diagnosis of brucellosis was confirmed via serological and imaging examinations. After combined anti-infective therapy and anti-inflammatory treatment, the patient’s condition alleviated. The results indicate that symptom overlap between UC and brucellosis can easily lead to misdiagnosis, highlighting the importance of detailed epidemiological history inquiry and targeted serological testing. Treatment should balance anti-infection and inflammation control, with individualized medication and multidisciplinary collaboration being particularly crucial.
