Abstract: Background: Patients with cleft lip and palate often present with dental defects and malocclusion. This report describes the clinical course of a patient with skeletal Class III high-angle malocclusion resulting from cleft lip and palate who underwent combined orthodontic and restorative treatment. Results: Following treatment, the patient’s facial profile improved, the dentition was aligned, dental integrity was restored, and the patient’s self-confidence increased. Conclusion: This case suggests that for cleft lip and palate patients who have not undergone bone grafting, combined orthodontic and restorative treatment can yield certain positive outcomes, including facial profile improvement and increased dental alignment. However, it should be noted that the treatment has limitations: the patient’s high-angle facial profile tends to worsen, the aesthetic effect of the smile is suboptimal, and the occlusal relationship is restored in a compromised manner.
