摘要: 长期以来,重症肺炎都是重症监护病房(ICU)中最常见的严重感染性疾病之一,尽管对大多数重症肺炎患者采取了积极的抗感染及器官支持治疗,但其病死率仍维持在20%~50%的较高水平。随着重症医学技术的进步以及人口老龄化的加剧,越来越多的重症肺炎患者得以存活出院,约30%的患者会进展为持续性炎症–免疫抑制–分解代谢综合征(persistent inflammation, immunosuppression and catabolism syndrome, PICS)。重症肺炎后PICS以持续低度炎症、免疫抑制及分解代谢亢进为特征,导致患者长期机械通气依赖、反复感染、肌肉萎缩及认知功能障碍,严重影响长期预后和生活质量。本文旨在系统阐述重症肺炎后PICS的发生机制、诊断标准、预测生物标志物及干预策略的最新研究进展,为临床早期识别高危患者、制定个体化干预策略提供理论依据。
Abstract: For a long time, severe pneumonia has been one of the most common severe infectious diseases in the intensive care unit (ICU). Despite the implementation of active anti-infection and organ support treatments for most patients with severe pneumonia, the mortality rate still remains at a relatively high level of 20% to 50%. With the advancement of critical care medicine and the intensification of population aging, more and more patients with severe pneumonia have been able to survive and be discharged from the hospital. Approximately 30% of these patients will progress to persistent inflammatory-immune suppression-degradation syndrome (PICS). PICS after severe pneumonia is characterized by persistent low-level inflammation, immune suppression, and hypermetabolic decomposition, leading to long-term mechanical ventilation dependence, repeated infections, muscle atrophy, and cognitive dysfunction, which seriously affects long-term prognosis and quality of life. This article aims to systematically review the latest research progress in the mechanisms, diagnostic criteria, predictive biomarkers, and intervention strategies of PICS after severe pneumonia, providing a theoretical basis for the early identification of high-risk patients and the formulation of individualized intervention strategies in clinical practice.
