重复经颅磁刺激治疗持续性姿势–感知性头晕患者的临床效果
Clinical Effect of Repetitive Transcranial Magnetic Stimulation in Patients with Persistent Postural-Perceptual Dizziness
摘要: 持续性姿势感性头晕(PPPD)属于一种常见的慢性功能前庭障碍,以持续的非旋转性头晕、姿势不稳定为特点,常伴焦虑、恐惧等心理症状,严重干扰患者生活。目前临床上常规治疗手段前庭康复训练、药物治疗、认知行为疗法等效果不理想,部分患者不能取得明显的改善。重复经颅磁刺激(Repetitive Transcranial Magnetic Stimulation, rTMS)作为一种无创、安全的神经调控技术,近年来被逐渐应用于PPPD的临床治疗,且取得了一定的疗效。本文在近几年国内外相关研究的基础上,对rTMS治疗PPPD的临床疗效、作用机制进行系统的综述,分析目前研究存在的不足以及未来的研究方向,为临床治疗PPPD提供理论依据和实践参考。
Abstract: Persistent Postural-Perceptual Dizziness (PPPD) is a common chronic functional vestibular disorder, characterized by persistent non-rotatory dizziness and postural instability, often accompanied by psychological symptoms such as anxiety and fear, which seriously interferes with patients’ quality of life. At present, the conventional clinical treatment methods, including vestibular rehabilitation training, drug therapy, and cognitive behavioral therapy, have unsatisfactory effects, and some patients cannot achieve significant improvement. Repetitive Transcranial Magnetic Stimulation (rTMS), as a non-invasive and safe neuromodulation technique, has been gradually applied in the clinical treatment of PPPD in recent years and achieved certain curative effects. Based on the relevant domestic and foreign studies in recent years, this article systematically reviews the clinical efficacy and mechanism of action of rTMS in the treatment of PPPD, analyzes the deficiencies of current studies and the future research directions, so as to provide a theoretical basis and practical reference for the clinical treatment of PPPD.
