摘要: 腰椎间盘突出症(LDH)是临床高发的脊柱退行性疾病,中医康复治疗手段多样但评估客观化不足、个体化精准度不高。DIERS formetric 3D作为一种无辐射、可量化的表面形貌脊柱三维评估系统,在脊柱侧凸评估中已获高质量证据验证,并逐步向退行性脊柱疾病延伸。医学运动康复(MTT)作为标准化主动康复体系,在改善腰椎稳定性、预防复发方面具有独特优势。本文系统回顾DIERS formetric 3D assessment system的技术原理、信效度研究进展及在脊柱疾病中的应用现状,总结MTT治疗LDH的循证医学证据,并基于中医“筋骨失衡、辨证论治”理论,探讨将二者整合入LDH中医精准康复体系的理论依据与实践路径。提出构建“无辐射评估–主动康复–中医药治疗”三位一体的精准康复模式,以期为LDH的中西医结合康复提供新思路。
Abstract: Lumbar disc herniation (LDH) is a common degenerative spinal disease that frequently affects middle-aged and older adults. While traditional Chinese medicine (TCM) offers various rehabilitation approaches, most of these rely heavily on subjective clinical assessments and lack objective, quantifiable evaluation tools. The DIERS formetric 3D system enables radiation-free, three-dimensional reconstruction of spinal morphology and has been validated for scoliosis evaluation. Medical training therapy (MTT) is a standardized active rehabilitation system that shows promise for improving lumbar stability and preventing recurrence. This review summarizes the technical principles, reliability, and validity of DIERS formetric 3D assessment system, along with the evidence supporting MTT for LDH rehabilitation. Drawing on the TCM theory of “tendon-bone disharmony” and syndrome differentiation, the review examines the rationale for integrating these methods into a precision rehabilitation model. A “radiation‑free assessment-active rehabilitation-TCM characteristic therapies” framework is proposed for LDH management.
