Abstract: Cushing’s syndrome (CS), characterized by chronic glucocorticoid excess due to dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, remains one of the most formidable diagnostic challenges in clinical endocrinology. Diagnostic precision is often compromised by episodic cortisol secretion, inter-individual variability, and technical disparities in laboratory assays, leading to significant rates of misdiagnosis. This review synthesizes recent advancements in CS diagnostics, specifically evaluating the clinical utility and limitations of 24-hour urinary free cortisol (UFC), late-night salivary cortisol (LNSC), low-dose dexamethasone suppression test (LDST), and hair cortisol analysis for qualitative screening. Furthermore, the diagnostic efficacy of imaging modalities, functional assays, and bilateral inferior petrosal sinus sampling (BIPSS) for etiological localization is analyzed. Emerging diagnostic technologies are also discussed. This paper aims to provide a standardized framework and practical evidence for the biochemical evaluation of Cushing’s syndrome in clinical practice.
