摘要: 脓毒症是由宿主对感染反应失调引发的危及生命的器官功能障碍,有预后差、病死率高等特点,明确脓毒症及其并发症的发生机制一直是亟待解决的问题。随着“微生物–肠–脑轴”双向调节机制进入人们视野,大量研究证明脓毒症及其并发症与该机制具有高度相关性,其中肠道微生物衍生的大量代谢物扮演了关键角色。本文聚焦于肠道微生物代谢物在脓毒症发生发展中的作用机制,系统综述这些代谢物如何通过调节肠道屏障功能、全身炎症反应、免疫细胞极化及远隔器官损伤等途径参与脓毒症的病理过程。旨在为该领域的深入研究提供理论依据,并探索基于代谢物的潜在治疗靶点。
Abstract: Sepsis is defined as a life-threatening organ dysfunction caused by a dysregulated host response to infection, characterized by poor prognosis and high mortality. Clarifying the mechanisms underlying sepsis and its complications have always been an urgent issue to be addressed. With the emergence of the bidirectional regulatory “microbiota - gut - brain axis” concept, a growing body of evidence has demonstrated a strong correlation between sepsis and its complications and this axis, in which gut microbiota-derived metabolites play a critical role. This review focuses on the mechanisms by which gut microbial metabolites contribute to the onset and progression of sepsis, systematically summarizing how these metabolites participate in the pathological processes of sepsis by modulating intestinal barrier function, systemic inflammatory responses, immune cell polarization, and remote organ injury. The aim is to provide a theoretical foundation for further research in this field and to explore metabolite-based potential therapeutic targets.
