摘要: 心肺运动试验(Cardiopulmonary Exercise Testing, CPET)作为一项动态、综合性的心肺评估工具,在急性冠脉综合征(Acute Coronary Syndrome, ACS)的综合管理中扮演着重要作用。本文通过综述近年来CPET在ACS领域的最新应用进展,重点分析其在诊断评估、风险分层及康复管理方面的独特价值。CPET通过对静息及运动状态下的气体代谢、呼吸、血压、心率、心电及症状的连续性监测,核心指标如峰值摄氧量(VO2-peak)、氧脉搏(O2 pulse)、无氧阈(AT)及二氧化碳通气当量斜率(VE/VCO2 slope)等可敏感反映心肌缺血导致的心肺功能损害,且对疾病预后有独立预测价值,是预测主要不良心血管事件(Major adverse cardiovascular events, MACE)和全因死亡的独立危险因素。目前CPET在ACS患者综合管理中展现出不可替代的作用,但未来仍需进一步开展相关研究,以推动CPET在ACS综合管理中的规范化与精准化应用。
Abstract: Cardiopulmonary exercise testing (CPET), as a dynamic and comprehensive assessment tool for the heart and lungs, plays a significant role in the integrated management of acute coronary syndrome (ACS). This article reviews the latest application progress of CPET in the field of ACS, with a focus on analyzing its unique value in diagnosis and assessment, risk stratification, and rehabilitation management. CPET continuously monitors gas metabolism, respiration, blood pressure, heart rate, electrocardiogram, and symptoms during both rest and exercise. Core indicators such as peak oxygen uptake (VO2-peak), oxygen pulse (O2 pulse), anaerobic threshold (AT), and carbon dioxide ventilation equivalent slope (VE/VCO2 slope) can sensitively reflect the damage to cardiopulmonary function caused by myocardial ischemia and have independent predictive value for disease prognosis. They are independent risk factors for major adverse cardiovascular events (MACE) and all-cause mortality. Currently, CPET has shown an irreplaceable role in the comprehensive management of ACS patients, but further research is still needed in the future to promote the standardized and precise application of CPET in the integrated management of ACS.
