腔镜甲状腺手术常见入路的研究进展

摘要: 甲状腺癌是内分泌系统最常见的恶性肿瘤,近年来随着发病率逐年上升,以及患者对美容要求的提高,传统开放手术遗留的颈部瘢痕难以满足部分患者的心理需求,腔镜甲状腺手术应运而生,同时逐步衍生出多种手术入路。近年来,经胸乳,经腋窝,经锁骨,经口腔,经颏下等手术入路不断改进与发展。目前各种手术入路在手术效能,术后恢复,美容满意度方面各有优劣,尚无一种入路能完全替代其他入路或适用于所有患者。本文按各手术入路的优缺点对国内外腔镜甲状腺手术的常见入路做一综述。
Abstract: Thyroid cancer is the most common malignant tumour of the endocrine system. In recent years, with incidence rates rising year on year and patients’ aesthetic expectations increasing, the neck scars left by traditional open surgery have failed to meet the psychological needs of some patients. Laparoscopic thyroid surgery has consequently emerged, giving rise to a variety of surgical approaches. In recent years, surgical approaches such as ETBA, ETGUA, TOETVA, GTAET and ETSA have undergone continuous refinement and development. Currently, the various surgical approaches each have their own advantages and disadvantages in terms of surgical efficacy, postoperative recovery and cosmetic satisfaction; no single approach can completely replace the others or be suitable for all patients. This article provides a review of the common approaches to endoscopic thyroid surgery both domestically and internationally, based on the advantages and disadvantages of each surgical approach.
