增强型体外反搏对冠状动脉肌桥患者的 双心功能的影响
Effect of Enhanced External Counterpulsation on Biventricular Function in Patients with Coronary Artery Myocardial Bridge
摘要: 目的:探讨增强型体外反搏(EECP)联合常规药物治疗对冠状动脉肌桥(MB)患者心脏功能、焦虑、抑郁状态的综合影响,评估其在治疗中的临床价值。方法:采用简单随机抽样法选取2024年2月至2025年2月确诊的90例MB患者,随机分为药物组(常规药物)、EECP组(单纯EECP)和联合组(药物 + EECP),每组30例。EECP治疗36小时为1疗程,至少2疗程。治疗前后比较心肺运动试验指标、西雅图心绞痛量表(SAQ)、广泛性焦虑障碍量表(GAD-7)、患者健康问卷抑郁量表(PHQ-9)、匹兹堡睡眠质量指数(PSQI)。结果:治疗后,联合组运动负荷时间、每搏氧量(VO2/HR)、代谢当量(MET)显著优于药物组和EECP组(P < 0.05);联合组GAD-7、PHQ-9及PSQI评分显著低于其他两组(P < 0.05),三组均无严重不良事件。结论:EECP联合药物治疗可显著改善MB患者的心功能、焦虑抑郁状态,实现心脏与心理的双重获益,具有良好的安全性。
Abstract: Objective: To investigate the comprehensive effects of enhanced external counterpulsation (EECP) combined with conventional drug therapy on cardiac function, anxiety/depression status, and cognitive function in patients with coronary artery myocardial bridging (MB), and to evaluate its clinical value in “dual-heart therapy”. Methods: From February 2024 to February 2025, 90 MB patients diagnosed were randomly selected using simple random sampling and divided into three groups: medication group (conventional drugs only), EECP group (EECP alone), and combined group (drugs + EECP), each comprising 30 patients. One course of EECP treatment consisted of 36 hours, with at least two courses administered. Cardiopulmonary exercise test parameters, Seattle Angina Questionnaire (SAQ), Generalized Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), and Pittsburgh Sleep Quality Index (PSQI) were compared before and after treatment. Results: After treatment, the combined group showed significantly better results than the medication and EECP groups in terms of exercise duration, stroke volume oxygen (VO2/HR), and metabolic equivalents (METs) (P < 0.05). The GAD-7, PHQ-9, and PSQI scores in the combined group were also significantly lower than those in the other two groups (P < 0.05). No serious adverse events occurred in any of the groups. Conclusion: EECP combined with drug therapy significantly improves cardiac function and alleviates anxiety and depression in MB patients, achieving dual benefits for both heart and mental health, with good safety profile.
