摘要: 目的:探究慢性阻塞性肺病(chronic obstructive pulmonary disease, COPD)患者合并感染鲍曼不动杆菌(Acinetobacter baumannii, AB)的高危因素,以期为临床早期识别高危患者并实施有效干预措施提供数据支撑。方法:选取2019年1月至2026年1月江西省吉安市中心人民医院呼吸与危重症医学科及重症医学科收治的1280例COPD患者作为研究对象,采用单因素和多因素Logistic回归分析相关危险因素。结果:多因素分析结果指出,短期内住院史、糖皮质激素及抗生素药物使用情况、有创侵入性操作、肺功能水平分级、急性加重频率、慢性基础病和年龄是COPD患者AB感染的独立危险因素。结论:有创侵入性操作、短期内住院史、肺功能水平分级均是COPD患者感染AB的关键危险因素。基于以上危险因素建立的个体化预测模型有助于临床工作早期识别高危患者,并及时采取针对性防控措施。
Abstract: Objective: To explore the high-risk factors of combined infection with Acinetobacter baumannii (AB) in patients with chronic obstructive pulmonary disease (COPD), so as to provide data support for the early clinical identification of high-risk patients and the implementation of effective intervention measures. Methods: This study was conducted on 1280 patients of COPD admitted to the Deptt of Respiratory and Critical Care Medicine and Department of Critical Care Medicine, Ji’an Central People’s Hospital, Jiangxi Province from January 2019 to January 2026. The related risk factors were identified through the univariate and multivariate logistic regression analyses. Results: The results of the multivariate analysis demonstrated that a history of short-term hospitalization, a history of glucocorticoid and antibiotic utilization, invasive procedures, the level of pulmonary function, the frequency of acute exacerbation, chronic underlying diseases, and age were independent risk factors for AB infection in patients with COPD. Conclusion: Invasive and intrusive medical procedures, a history of short-term hospitalization, and the classification of pulmonary function levels are all crucial risk factors for patients with COPD to be infected with AB. The individualized prediction model developed based on these risk factors is beneficial for the early identification of high-risk patients in clinical practice and the prompt implementation of targeted prevention and control measures.
