流行性乙型脑炎继发吉兰–巴雷综合征一例

摘要: 流行性乙型脑炎(Japanese Encephalitis, JE)是由乙脑病毒(Japanese Encephalitis Virus, JEV)引起的蚊媒人畜共患病,近年成人发病比例呈上升趋势。吉兰–巴雷综合征(Guillain-Barré Syndrome, GBS)是感染诱发的免疫介导性急性周围神经病。本文报道一例JEV感染继发GBS,经JE标准治疗后出现对称性四肢及眼睑肌无力,确诊急性运动感觉轴索型GBS。本病例提示,JE病程中出现肢体无力需警惕是否继发GBS,及时行神经电生理检查有助于明确诊断,以期减少漏诊、改善患者预后。
Abstract: Japanese Encephalitis (JE) is a mosquito-borne zoonosis caused by the Japanese Encephalitis Virus (JEV), with an increasing proportion of adult cases in recent years. Guillain-Barré Syndrome (GBS) is an infection-induced immune-mediated acute peripheral neuropathy. We report a case of GBS secondary to JEV infection. The patient developed symmetric limb and eyelid weakness after standard JE treatment, and was diagnosed with acute motor-sensory axonal neuropathy (AMSAN) subtype of GBS. This case suggests that limb weakness during JE should raise suspicion for secondary GBS. Timely neuroelectrophysiological examination is helpful to confirm the diagnosis, so as to reduce missed diagnosis and improve the prognosis of patients.
