老年糖尿病患者抑郁与焦虑共病的研究进展及干预策略
Research Progress and Intervention Strategies for Depression-Anxiety Comorbidity in Elderly Diabetic Patients
摘要: 老年糖尿病患者常伴随抑郁和焦虑等心理问题,其发生不但影响血糖控制,还会降低患者生活质量并增加并发症风险。本文围绕老年糖尿病患者心理问题现状、相关危险因素、可能机制及干预进展进行综述,重点归纳心理因素与代谢控制之间的相互作用,强调早期识别与多学科综合干预的重要性。并结合老年群体特征,进一步讨论了衰弱、认知障碍、多病共存、用药复杂性及社会支持不足等老年特异性问题,并从中国家庭结构、社区医疗资源与基层卫生服务现状出发,提出更符合本土情境的综合干预思路。但未来仍需加强高质量研究,进一步探索适用于老年患者的个体化、持续性心理干预模式,以提升疾病管理效果和缓解疾病进程。
Abstract: Elderly patients with diabetes often suffer from psychological issues such as depression and anxiety, which not only affect blood glucose control but also reduce their quality of life and increase the risk of complications. This article provides a review of the current status of psychological problems in elderly diabetic patients, related risk factors, potential mechanisms, and progress in interventions, focusing on summarizing the interaction between psychological factors and metabolic control while emphasizing the importance of early identification and multidisciplinary comprehensive intervention. Furthermore, considering the characteristics of the elderly population, it discusses age-specific issues such as frailty, cognitive impairment, comorbidities, medication complexity, and insufficient social support. Based on China’s family structure, community healthcare resources, and the current state of primary healthcare services, the article proposes comprehensive intervention strategies better suited to local contexts. However, future research should focus on conducting high-quality studies to further explore personalized and sustained psychological intervention models tailored for elderly patients, thereby improving disease management outcomes and slowing disease progression.
