摘要: 当前全球代谢性疾病发病率持续攀升,传统降糖、调脂类药物多为单一靶点干预,难以同步改善糖脂紊乱、体重异常等多重问题。替尔泊肽作为首个获批的GIP/GLP‑1双受体激动剂,依托胰高糖素样肽‑1 (GLP‑1)受体与葡萄糖依赖性促胰岛素多肽(GIP)受体的协同激活效应,在代谢性疾病治疗中展现出多方面作用。近年来多项临床研究结果显示,其在2型糖尿病、肥胖症、代谢相关脂肪性肝病、阻塞性睡眠呼吸暂停等代谢相关疾病中,可实现降糖、减重、改善胰岛素抵抗、调节脂代谢以及抑制慢性炎症等多重效果。本文系统梳理替尔泊肽的作用机制与临床研究进展,总结代谢疾病领域探索方向,为临床合理应用提供参考。
Abstract: The global incidence of metabolic diseases continues to rise. Traditional antidiabetic and lipid-modulating drugs typically target a single pathway, making it difficult to simultaneously address multiple issues such as glucose-lipid disorders and abnormal body weight. Tirzepatide, the first approved dual GIP/GLP-1 receptor agonist, exerts broad therapeutic effects in metabolic diseases through the synergistic activation of both glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) receptors. Recent clinical studies have shown that tirzepatide can effectively lower blood glucose, promote weight loss, improve insulin resistance, regulate lipid metabolism, and suppress chronic inflammation in conditions including type 2 diabetes, obesity, metabolic-associated fatty liver disease, and obstructive sleep apnea. This article systematically reviews the mechanism of action and clinical research progress of tirzepatide, summarizes key directions in metabolic disease research, and provides insights for its rational clinical application.
