摘要: 随着全球人口老龄化进程加快及糖尿病患病率持续升高,2型糖尿病(T2DM)合并认知功能障碍的患病率呈逐年上升趋势,已成为糖尿病慢性并发症管理中的重难点。认知功能障碍不仅影响患者的自我管理能力、治疗依从性及生活质量,还可增加低血糖、跌倒及痴呆等不良结局风险。近年来国内外的多项研究表明,T2DM患者认知功能障碍的发生受多种因素共同影响。本文旨在通过多维视角,系统综述国内外关于T2DM患者认知功能障碍影响因素的研究现状,主要涉及人口学因素、糖代谢异常、血管危险因素、糖尿病并发症、心理社会因素及生活方式等多个维度。总结发现,认知功能障碍是多因素共同作用的结果,未来研究应聚焦于建立多学科协作的早期识别预警模型,并探索针对可干预因素的个体化护理干预策略,以期为延缓T2DM患者认知功能衰退提供科学依据。
Abstract: With the accelerating global population aging process and the continuous increase in the prevalence of diabetes, the prevalence of type 2 diabetes mellitus (T2DM) complicated with cognitive impairment has been rising year by year, which has become a key and difficult point in the management of chronic diabetic complications. Cognitive impairment not only affects patients’ self-management ability, treatment compliance, and quality of life but also increases the risk of adverse outcomes such as hypoglycemia, falls, and dementia. In recent years, numerous domestic and international studies have shown that the occurrence of cognitive impairment in T2DM patients is influenced by multiple factors. This paper aims to systematically review the current research status of the influencing factors of cognitive impairment in T2DM patients from a multi-dimensional perspective, mainly covering multiple dimensions such as demographic factors, abnormal glucose metabolism, vascular risk factors, diabetic complications, psychosocial factors, and lifestyle. The summary reveals that cognitive impairment is the result of the combined action of multiple factors. Future research should focus on establishing an early identification and warning model through multidisciplinary collaboration and exploring individualized nursing intervention strategies targeting modifiable factors, with the aim of providing a scientific basis for delaying the cognitive decline of T2DM patients.
