Sonazoid超声造影与增强CT对肝占位性病变 诊断价值的比较研究

摘要: 目的:探讨示卓安(Sonazoid)超声造影与增强CT在肝脏占位性病变诊断中的临床应用价值。方法:回顾性分析2023年2月~2025年2月日照市人民医院收治的疑似肝脏占位性病变66例患者的临床及影像学资料。所有患者均行超声造影和增强CT检查,并以患者的穿刺或手术病理诊断结果为诊断金标准,比较这两种检查方式在诊断肝脏占位性病变的准确度以及对其良恶性鉴别准确率。结果:以穿刺及手术病理结果为金标准,经Sonazoid超声造影和增强CT诊断肝脏占位性病变患者的准确率分别为78.79% (52/66)、71.21% (47/66),其诊断恶性病变的准确率分别为78.18% (43/55)、72.72% (40/55),其诊断良性病变的准确率分别为81.81% (9/11)、63.63% (7/11)。Sonazoid超声造影与增强CT在肝脏占位性病变良恶性鉴别的比较,结果显示超声造影的灵敏度(78.18% vs 72.72%)优于增强CT,两者间比较差异具有统计学意义(P < 0.05),但其特异度的比较两者间差异无明显统计学意义(P > 0.05)。结论:超声造影及增强CT均可对肝脏占位性病变的诊断提供可靠依据;Sonazoid的超声造影对诊断肝脏恶性占位性病变的灵敏度优于增强CT,相对而言,Sonazoid超声造影因具有更加便捷、安全、经济等优势,更适合大范围临床应用。
Abstract: Objective: To evaluate the clinical value of Sonazoid contrast-enhanced ultrasound (CEUS) versus contrast-enhanced computed tomography (CECT) in the diagnosis of hepatic space-occupying lesions. Methods: Clinical and imaging data of 66 patients with suspected hepatic space-occupying lesions admitted to Rizhao People’s Hospital from February 2023 to February 2025 were retrospectively analyzed. All patients underwent both CEUS (using Sonazoid) and CECT. Using pathological results from biopsy or surgery as the gold standard, the diagnostic accuracy of the two modalities for hepatic space-occupying lesions and their accuracy in differentiating benign from malignant lesions were compared. Results: With pathology as the reference standard, the diagnostic accuracy of Sonazoid CEUS and CECT for hepatic space-occupying lesions was 78.79% (52/66) and 71.21% (47/66), respectively. The accuracy for malignant lesions was 78.18% (43/55) for CEUS and 72.72% (40/55) for CECT; the accuracy for benign lesions was 81.81% (9/11) for CEUS and 63.63% (7/11) for CECT. In differentiating benign from malignant hepatic lesions, the sensitivity of Sonazoid CEUS was superior to that of CECT (78.18% vs 72.72%), and the difference was statistically significant (P < 0.05). However, no statistically significant difference was observed in specificity between the two methods (P > 0.05). Conclusion: Both CEUS and CECT can provide reliable evidence for the diagnosis of hepatic space-occupying lesions. Sonazoid CEUS shows superior sensitivity to CECT in diagnosing malignant hepatic space-occupying lesions. Moreover, Sonazoid CEUS is more convenient, safer, and more cost-effective, making it more suitable for widespread clinical application.
