超声血流参数联合血清学指标在甲状腺乳头状癌颈部淋巴结转移中的诊断价值
Diagnostic Value of Ultrasound Hemodynamic Parameters Combined with Serological Indicators in Cervical Lymph Node Metastasis of Papillary Thyroid Carcinoma
摘要: 目的:探讨超声血流参数联合血清学指标在甲状腺乳头状癌颈部淋巴结转移中的诊断价值。方法:选取2022年1月至2024年12月于本院收治并经手术病理证实的PTC患者92例作为研究对象,根据术后病理结果分为转移组44例和未转移组48例。所有患者术前均接受常规超声、彩色多普勒超声及血清学检查,记录血流阻力指数(RI)、收缩期峰值流速(PSV)、舒张末期流速(EDV)以及甲状腺刺激素(TSH)、甲状腺球蛋白(TG)和甲状腺过氧化物酶抗体(TPOAb)等指标。采用受试者工作特征(ROC)曲线评价各参数对颈部淋巴结转移的诊断效能。结果:转移组患者RI、TSH及TG水平均高于未转移组,差异有统计学意义(P < 0.05);PSV、EDV及TPOAb比较差异无统计学意义(P > 0.05)。ROC曲线分析显示,RI对颈部淋巴结转移具有一定诊断价值,联合TSH、TG后诊断效能进一步提高。结论:超声血流参数联合血清学指标对PTC颈部淋巴结转移具有较好的诊断价值,其中RI、TSH及TG可作为评估颈部淋巴结转移风险的重要参考指标,具有一定临床应用前景。
Abstract: Objective: To investigate the diagnostic value of ultrasound hemodynamic parameters combined with serological indicators in predicting cervical lymph node metastasis in patients with papillary thyroid carcinoma (PTC). Methods: A total of 92 patients with PTC confirmed by postoperative pathology in our hospital from January 2022 to December 2024 were retrospectively enrolled. According to postoperative pathological results, the patients were divided into a metastasis group (n = 44) and a non-metastasis group (n = 48). All patients underwent conventional ultrasound, color Doppler ultrasound, and serological examinations before surgery. Resistance index (RI), peak systolic velocity (PSV), end-diastolic velocity (EDV), thyroid-stimulating hormone (TSH), thyroglobulin (TG), and thyroid peroxidase antibody (TPOAb) were recorded. Receiver operating characteristic (ROC) curves were used to evaluate the diagnostic performance of these parameters for cervical lymph node metastasis. Results: RI, TSH, and TG in the metastasis group were significantly higher than those in the non-metastasis group (P < 0.05), while PSV, EDV, and TPOAb showed no significant differences (P > 0.05). ROC analysis indicated that RI had certain diagnostic value, and the combined assessment with TSH and TG further improved diagnostic efficiency. Conclusion: Ultrasound hemodynamic parameters combined with serological indicators show good diagnostic value for cervical lymph node metastasis in PTC. RI, TSH, and TG may serve as important indicators for evaluating the risk of cervical lymph node metastasis.
