摘要: 结直肠恶性肿瘤肠造口患者中,肿瘤复发高危群体面临造口护理难题与病情反复双重压力,传统统一化护理模式缺乏针对性,护理效果有限。本文旨在探讨肿瘤复发高危造口患者分层护理策略的实践价值与教学心得。基于肿瘤分期、淋巴结转移、造口愈合、基础疾病等多维度指标,将患者划分为低度、中度、高度高危三层;从造口创面养护、健康指导、心理干预、延续防护四维度实施梯度化护理,并同步开展阶梯化实操教学。临床实践显示,分层护理可精准调配资源、降低造口并发症发生率、管控肿瘤复发诱因、提升患者康复质量。教学中可助力护理人员构建全周期一体化照护思维,强化综合评估与应急处置能力。综上,分层护理策略适配高危造口患者个性化需求,兼具临床实践价值与专科教学意义,值得推广应用。
Abstract: Patients with enterostomy after colorectal malignant tumor surgery who are at high risk of tumor recurrence face dual pressures from stoma care difficulties and recurrent conditions. The traditional unified nursing model lacks pertinence with limited nursing effects. This paper aims to explore the practical value and teaching experience of stratified protective nursing strategies for ostomy patients at high risk of tumor recurrence. Based on multi-dimensional indicators such as tumor stage, lymph node metastasis, stoma healing and underlying diseases, patients were divided into three levels: low, moderate and high risk. Gradient nursing was implemented from four dimensions: stoma wound care, health guidance, psychological intervention and continuous protection, and stepped practical teaching was carried out simultaneously. Clinical practice shows that stratified protective nursing can accurately allocate resources, reduce the incidence of stoma complications, control tumor recurrence incentives and improve patients’ rehabilitation quality. In teaching, it can help nursing staff construct a full-cycle integrated care thinking and strengthen comprehensive assessment and emergency response capabilities. In conclusion, the stratified protective nursing strategy meets the personalized needs of high-risk ostomy patients, has both clinical practice value and professional teaching significance, and is worthy of promotion and application.
