摘要: 糖尿病视网膜病变(DR)作为世界第一大致盲性眼底疾病,发病率仍在不断上升,严重影响患者的视觉和生活质量。糖尿病黄斑水肿(DME)是DR患者视力受损的主要原因,具有复杂的病理生理机制,其中血管内皮生长因子(VEGF)在DR与DME的发病机制中起着举足轻重的作用。因此,探索应用抗VEGF药物的最新发展尤为重要。本文旨在叙述近年来抗VEGF药物在DME治疗中的研究进展,分析其作用机制、药代动力学特点,并基于临床试验数据,比较其在糖尿病黄斑水肿治疗中的疗效、持久性、治疗周期及安全性,总结各药物的优势、劣势及适用人群,并展望未来的研究方向,以期为临床个体化治疗提供更为全面的参考依据。
Abstract: Diabetic retinopathy (DR) is the leading cause of blindness worldwide, and its incidence continues to rise, severely affecting patients’ vision and quality of life. Diabetic macular edema (DME) is the primary cause of visual impairment in patients with DR and involves complex pathophysiological mechanisms, in which vascular endothelial growth factor (VEGF) plays a pivotal role in the pathogenesis of both DR and DME. Therefore, exploring the latest developments in the application of anti-VEGF drugs is particularly important. This article aims to review recent research progress on anti-VEGF drugs in the treatment of DME, analyze their mechanisms of action and pharmacokinetic characteristics, and, based on clinical trial data, compare their efficacy, durability, treatment duration, and safety in the management of diabetic macular edema. It summarizes the advantages, disadvantages, and target patient populations of each drug and outlines future research directions, with the goal of providing a more comprehensive reference for personalized clinical treatment.
