摘要: 磷丙泊酚二钠(fospropofol disodium)是我国自主研发的水溶性丙泊酚前体药物,目前已获批用于成人全身麻醉诱导与维持、重症监护病房(ICU)机械通气患者镇静及非气管插管诊疗操作镇静。该药经体内碱性磷酸酶水解释放活性代谢产物丙泊酚,具有起效相对平缓、血药浓度上升稳定等药代动力学特点。与传统脂肪乳剂型丙泊酚相比,其在降低注射痛发生率、减轻循环和呼吸抑制方面具有一定优势。然而,该药仍存在起效时间延长、感觉异常及瘙痒发生率较高等问题。本文就磷丙泊酚二钠的药理学特征、临床应用现状及安全性研究进展进行综述,并对其未来研究方向进行展望,以期为临床合理应用提供参考。
Abstract: Fospropofol disodium is a novel water-soluble prodrug of propofol independently developed in China. It has been approved for the induction and maintenance of general anesthesia in adults, sedation of mechanically ventilated patients in the intensive care unit (ICU), and sedation during non-intubated diagnostic and therapeutic procedures. Following intravenous administration, fospropofol disodium is hydrolyzed by alkaline phosphatase to release the active metabolite propofol, exhibiting pharmacokinetic characteristics such as a relatively gradual onset of action and stable increases in plasma drug concentrations. Compared with conventional lipid-emulsion propofol formulations, fospropofol disodium demonstrates advantages in reducing injection pain and attenuating cardiovascular and respiratory depression. However, its clinical use is still associated with certain limitations, including a delayed onset of action and a relatively high incidence of paresthesia and pruritus. This review summarizes the pharmacological characteristics, current clinical applications, and safety profile of fospropofol disodium, and discusses future research directions, with the aim of providing a reference for its rational clinical use.
