Research Progress and Prospects of Adductor Canal Block in Elderly Orthopedic Surgery
摘要: 老年骨科手术患者常伴多种合并症,传统阿片类药物镇痛在老年患者中易引发呼吸抑制、认知功能障碍等不良反应。收肌管阻滞可选择性阻断隐神经,在有效镇痛的同时保留股四头肌肌力,减少阿片用量并降低跌倒风险,在老年骨科中优势独特。本文系统综述了ACB的解剖基础、临床应用、与其他技术比较、安全性及未来方向,旨在为临床个体化镇痛提供参考。
Abstract: Elderly orthopedic surgery patients often have multiple comorbidities, and traditional opioid analgesia in elderly patients is prone to adverse reactions such as respiratory depression and cognitive dysfunction. Adductor canal block can selectively block the saphenous nerve, effectively providing analgesia while preserving quadriceps muscle strength, reducing opioid use and lowering the risk of falls, offering unique advantages in geriatric orthopedics. This article systematically reviews the anatomical basis, clinical applications, comparisons with other techniques, safety, and future directions of ACB, aiming to provide a reference for individualized clinical analgesia.
