数智化转型下智慧社区体医 融合老年健康服务体系的 构建与优化路径

The Construction and Optimization Path of the Integrated Elderly Health Service System of Smart Community Physical Medicine under the Transformation of Digital Intelligence

摘要: 随着“健康中国2030”战略不断向前推进、智慧社区不断发展、人口老龄化程度持续加深的背景环境当中，体医融合服务已然成为促进老年健康的关键途径。此项研究建立起了基于可穿戴设备的智慧社区体医融合健康管理模式，着力解决当前社区健康服务里体医资源相互分离、健康监测不连贯、个性化供给不足等问题，为社区老年健康管理提供可参考的技术方法和实践经验。该研究搭建了可穿戴设备与手机端交互平台，达成健康数据的实时采集与反馈，形成了契合社区场景的体医融合服务模式。研究表明这种模式能够提高社区老年健康管理效率，为同类智慧社区体医融合建设提供可供参考的路径。

Abstract: With the continuous advancement of the “Healthy China 2030” strategy, the continuous development of smart communities, and the continuous deepening of population aging, physical and medical integration services have become a key way to promote the health of the elderly. This research has established a smart community-medical integrated health management model based on wearable devices, and strives to solve the problems of separation of physical and medical resources, incoherence of health monitoring, and insufficient personalized supply in the current community health services, and provides technical support and practical solutions for the health management of the elderly in the community. The research has built an interactive platform between wearable devices and mobile phones to achieve real-time collection and feedback of health data, forming a physical and medical integration service model suitable for community scenarios. Research shows that this model can improve the efficiency of elderly health management in the community and provide a reference path for the integration of physical medicine in similar intelligent communities.
